目的 探讨富含半胱氨酸的酸性分泌蛋白(SPARC)对2型糖尿病视网膜病变(DR)的影响.方法 临床病例对照研究.对2013年6~12月,在保定市第一中心医院,通过眼底荧光造影或眼底照相检查,将100例2型糖尿病患者分为2组:糖尿病不伴视网膜病变组(NDR) 38例;背景型视网膜病变组(BDR组)31例;增生型视网膜病变组(PDR组)31例.选取健康体检者42名作为对照组(CON组).检测血清SPARC浓度及空腹血糖(FBG)、糖化血红蛋白(HbA1C)、血脂等.结果 糖尿病视网膜病变组血清SPARC水平(18.98+3.35) μg/L高于糖尿病不伴视网膜病变组(16.26+2.07) μg/L及对照组(14.85+2.05) μg/L,差异有统计学意义(F=22.04,P<0.01).增生型视网膜病变组血清SPARC水平(19.86+3.60) μg/L高于背景型视网膜病变组(17.88+3.19) μg/L,差异有统计学意义(t =3.57,P<0.01).多元回归分析显示:LDL-C、FBG、HbA1C是血清SPARC的独立危险因素.结论 血清SPARC是糖尿病视网膜病变发生的危险因素.

Objective To discuss the influence of serum secreted protein acidic and rich in cysteine (SPARC) on the pathogenesis of diabetic retinopathy (DR).Methods All the patients had routine FFA and examination of fundus photos.A total of 100 patients of T2DM were divided into three groups:No signs of diabetic retinopathy (NDR group n=38),Background DR (BDR group n=31),proliferative DR (PDR group n=31).Forty-two persons were served as control group (CON group).The serum levels of SPARC,fasting blood glucose,glycosylated hemoglobin and the blood lipids were measured.Results Serum SPARC levels were significantly higher in DR group than those in NDR group and CON group.In DR group,serum SPARC levels in PDR group were significantly higher than that in BDR group.By stepwise regression analysis,LDL-C,FBG,HbA1C were independent risk factors for serum SPARC level in type 2 diabetes.Conclusions The serum SPARC may be a risk factor for the occurrence of DR.
