目的 探讨人类不同年龄组正常主涎腺磁共振成像(MRI)体积、信号强度及表观扩散系数(ADC)值的增龄变化规律.方法 回顾分析2009至2014年间苏州大学附属第二医院行常规头颈部MRI检查、无涎腺疾病的患者300例,测量4个年龄组共300例受试者600侧腮腺、颌下腺、舌下腺的体积、T1WI、T2WI信号强度与ADC值.以脑脊液为对照,计算各涎腺的相对信号强度(RSI).结果 腮腺的体积、T1WI RSI、T2WI RSI和舌下腺的T2WI RSI与年龄有相关性(P<0.01).腮腺体积、T1WI RSI、T2WI RSI在老年期之前逐渐升高,老年期后有所下降,其中未成年组与中年组的差异有统计学意义(P<0.01).只有未成年组与中年组颌下腺的ADC值差异有统计学意义(P<0.01).结论 MRI能有效评价正常涎腺结构与功能的增龄性改变,可以为老龄化研究和涎腺疾病的评估提供基线参考依据,具有较高的临床价值.

Objective To analyze the age-related characteristics of volumes,signal intensities (SIs) of T1-weighted images (T1WI),T2-weighted images (T2WI) and apparent diffusion coefficients (ADCs) of three major salivary glands.Methods A total of 300 subjects with normal salivary glands were divided into 4 different age groups and examined with a 1.5 T magnetic resonance (MR) unit.T1 WI,T2WI and diffusion-weighted MR images (DW MRI) were obtained and bilateral parotid,submandibular and sublingual glands segmented manually.The volumes,T1 WI,T2 WI SIs and ADCs of three major salivary glands were measured.And the relative SIs (RSIs) were assessed by comparing with cerebrospinal fluid.Results The volumes,T1WI and T2WI RSIs of parotid glands were significantly correlated with age (P <0.01) and showed similar significant tendencies of growing from minority to middle age and decreasing slightly after old age.Significant differences existed between minority and middle age groups in all abovementioned parameters (P < 0.01).The ADCs of submandibular glands also showed that there was a significant difference between the minority and middle age groups (P < 0.01).Conclusion MRI may be applied for studying the morphological and functional changes of normal major salivary glands with aging.Thus clinical rationales can be provided for assessing the subjects of any age during salivary gland imaging and aging-related researches.
