目的 探讨超声造影在脑肿瘤显微手术中的安全性和指导作用.方法 2011年6月至2012年10月行手术治疗的脑肿瘤患者43例,术中均行超声造影识别肿瘤边界,判断肿瘤良恶性程度指导手术切除.手术残腔内再次行超声造影判定肿瘤残余.结果 所有肿瘤在超声造影显像下均定位准确,边界清楚.不同性质肿瘤注入造影剂后出现不同程度强化,其强化的时间、强度和分布不同(P<0.05).超声造影共判定肿瘤残余19处,病理证实其中18处为肿瘤残余,1处为凝血块.结论 超声造影有助于准确识别肿瘤边界,判断肿瘤良恶性程度,发现肿瘤残余,引导手术中最大程度地切除肿瘤.

목적 탐토초성조영재뇌종류현미수술중적안전성화지도작용.방법 2011년6월지2012년10월행수술치료적뇌종류환자43례,술중균행초성조영식별종류변계,판단종류량악성정도지도수술절제.수술잔강내재차행초성조영판정종류잔여.결과 소유종류재초성조영현상하균정위준학,변계청초.불동성질종류주입조영제후출현불동정도강화,기강화적시간、강도화분포불동(P<0.05).초성조영공판정종류잔여19처,병리증실기중18처위종류잔여,1처위응혈괴.결론 초성조영유조우준학식별종류변계,판단종류량악성정도,발현종류잔여,인도수술중최대정도지절제종류.
Objective To discuss the safety and guiding effect of contrast-enhanced ultrasound(CEUS) in the resection of brain tumors.Methods Forty-three patients who were doubted brain tumors by computed tomography(CT) or magnetic resonance (MR) imaging were included in this study.All cases received microsurgical operation to remove the tumors using CEUS.Surgeon could determine the malignancy degree of the tumor and identify the tumor boundary according to the characteristics of ultrasound contrast imaging.After the resection of the tumor,CEUS was performed again in the surgical cavity to judge residual tumor.The suspicious tissues were removed and did pathological examination to clarify the nature of the tumor.Results Total tumor resection was achieved in thirty-five cases,subtotal in eight.The boundary of all lesions was well displayed under CEUS.Each tumor had different ultrasound performance in lesion morphology,boundary,internal echo,peripheral edema,internal flow and other aspects.All tumors after injecting ultrasound contrast agent (UCA) appeared different levels of enhancement and had different intensifying time,intensity,and distribution(P < 0.05).Eighteen residual tumors were judged through CEUS after the resection,which were confirmed by pathology tests later.Conclusion CEUS in brain tumors microsurgery could help to estimate tumor grade,identify tumor boundary accurately,and find residual tumor,which could guide to remove the tumor maximally.
