目的探讨1 470 nm半导体激光汽化术治疗良性前列腺增生的安全性及疗效.方法 2012年1月至2013年1月,我院应用1 470 nm半导体激光对87例良性前列腺增生患者行激光汽化术.患者年龄55～82岁,前列腺体积为29.7～83.9 ml,平均(52.4±8.2) ml.观察记录手术时间、出血情况、术前合并症及术后并发症,术后6个月内进行随访评估,采用统计学方法分析手术前后国际前列腺症状评分(international prostate symptom score,IPSS)、生命质量评分(quality of life,QOL)、最大尿流率(maximum urine flow rate,Qmax)及残余尿量(post void residual,PVR)等相关指标的差异.结果 87例手术均获得成功.手术时间35～104 min,平均(66±10) min.术中均未见明显出血,无输血病例.患者术后留置尿管时间20～70 h,平均(33±5)h.术后住院时间3～6d,平均(4±1)d.术后IPSS评分及QOL评分较术前改善,Qmax及PVR明显好于术前,手术前后比较差异均有统计学意义(P＜0.01). 结论 1 470 nm半导体激光汽化术是治疗良性前列腺增生安全有效的微创治疗方法.

Objective To explore the efficacy and safety of 1 470 nm diode laser vaporization in the treatment of benign prostatic hyperplasia (BPH).Methods Eighty-seven BPH patients were treated by 1 470 nm diode laser vaporization and followed up for 6 months.The patients were aged 55-82 years,with a mean prostate volume of (52.4±8.2) ml.The mean operation time,intra-operative blood loss,postoperative complications,international prostate symptom score (IPSS),quality of life (QOL) score,maximum urine flow rate (Qmax),and post void residual (PVR) before and after surgery were analyzed and compared.Results Operations were successful in all of the 87 cases,with an average operation time of (66± 10) min,little blood loss and no blood transfusion.The transurethral catheter indwelling time was 20-70 h (33±5 h).The patients were followed up for 6 months,which revealed a significant reduction in IPSS and QOL (P＜ 0.01) and improvement in Qmax and PVR (P ＜ 0.01) as compared with preoperative parameters.Conclusion Transurethral 1 470 nm diode laser vaporization is a safe and effective treatment for BPH.
