目的：初步探讨人根尖周炎中亲环素 A（CypA）表达及与临床表现的关系。方法：收集根尖手术切除并经病理诊断证实的根尖周炎病损组织83例作为实验组；收集新鲜拔除的健康正畸牙牙髓6例作为正常对照组。用免疫组织化学染色法分析比较各组织中 CypA 及 RANKL 的表达情况，取蛋白平均吸光度值（A值）进行统计学分析，并比较其表达量与临床症状的相关性。结果：根尖周炎病变组织中 CypA 主要表达于炎细胞浸润区和上皮组织，主要阳性细胞为淋巴样细胞、上皮细胞、血管内皮细胞，正常对照组中少见 CypA 的表达；CypA 在根尖肉芽肿组织中的表达量明显高于根尖囊肿组（P ＜0．05），RANKL 在根尖囊肿组织中的表达量明显高于根尖肉芽肿组（P ＜0．05）；有明显临床症状的根尖周炎组织中 CypA 和 RANKL 的 A 值均明显高于无症状组（P ＜0．05）；CypA 和 RANKL 的表达水平与根尖周炎病变大小均呈正相关（r ＝0．462，P ＜0．05；r ＝0．417，P ＜0．05）。结论：CypA 存在于人根尖周炎病损组织中，可能参与人根尖周炎疾病的发病进程。
목적：초보탐토인근첨주염중친배소 A（CypA）표체급여림상표현적관계。방법：수집근첨수술절제병경병리진단증실적근첨주염병손조직83례작위실험조；수집신선발제적건강정기아아수6례작위정상대조조。용면역조직화학염색법분석비교각조직중 CypA 급 RANKL 적표체정황，취단백평균흡광도치（A치）진행통계학분석，병비교기표체량여림상증상적상관성。결과：근첨주염병변조직중 CypA 주요표체우염세포침윤구화상피조직，주요양성세포위림파양세포、상피세포、혈관내피세포，정상대조조중소견 CypA 적표체；CypA 재근첨육아종조직중적표체량명현고우근첨낭종조（P ＜0．05），RANKL 재근첨낭종조직중적표체량명현고우근첨육아종조（P ＜0．05）；유명현림상증상적근첨주염조직중 CypA 화 RANKL 적 A 치균명현고우무증상조（P ＜0．05）；CypA 화 RANKL 적표체수평여근첨주염병변대소균정정상관（r ＝0．462，P ＜0．05；r ＝0．417，P ＜0．05）。결론：CypA 존재우인근첨주염병손조직중，가능삼여인근첨주염질병적발병진정。
AIM:To examine the expression of cyclophilin A (CypA)in human periapical lesions and to investigate its relationship with the clinical symptoms of periapical periodontitis.METHODS:Samples of human api-cal periodontitis lesions of 83 cases were subjected to immunohistochemical examination for CypA and RANKL expres-sion.Six 6 healthy dental pulp samples obtained from orthodontic patients served as the controls.The integrated aver-age optical density value (A)of CypA and RANKL was measured by immunohistochemical analysis for all the samples. Differences in CypA and RANKL expression between different clinical presentations were subsequently analyzed by t -test.RESULTS:CypA postitive cells and RANKL positive cells were both detected in all periapical lesion tissues, and the expression of CypA was significantly higher in periapical lesions than in normal tissues (P <0.05).Epithelial cells,lymphoid cells and endothelial-like cells showed CypA positive in the lesions.The expression of RANKL in cyst group were higher than in granuloma group (Z =4.48,P <0.05),while the expression of CypA in granuloma group were higher than in cyst group (Z =4.48,P <0.05).The average (A)value of CypA positive sites in periapical le-sions with clinical symptoms was higher than that without clinical symptoms(77.24 versus 39.18,P <0.05).Positive correlation was observed between CypA expression and lesion size(r =0.462,P <0.05).Moreover,positive correla-tion between RANKL expression and lesion size was observed (r =0.417,P <0.05).CONCLUSION:CypA might be involved in the inflammatory response and bone resorption,and associated with periapical lesion pathogenesis.
