目的：分析乳腺导管内乳头状瘤M RI表现特点及诊断价值。方法回顾性分析31例经手术病理证实的乳腺导管内乳头状瘤 MRI 表现，包括形态学、平扫信号、动态增强 TIC 形态、DWI 信号及 ADC 值特点。结果乳腺导管内乳头状瘤 MRI 表现分为三类：①囊状扩张的导管内见实性结节；②实性结节肿块型（伴或不伴邻近导管扩张）；③隐匿性乳头状瘤，既仅见扩张的导管，未见明确实性结节。乳腺导管内乳头状瘤的时间‐信号强度曲线与 ADC 值表现多样，其诊断需结合形态学。结论 MRI 对病灶形态及功能成像具有优势，可为临床确诊乳腺导管内乳头状瘤提供一定参考价值。
목적：분석유선도관내유두상류M RI표현특점급진단개치。방법회고성분석31례경수술병리증실적유선도관내유두상류 MRI 표현，포괄형태학、평소신호、동태증강 TIC 형태、DWI 신호급 ADC 치특점。결과유선도관내유두상류 MRI 표현분위삼류：①낭상확장적도관내견실성결절；②실성결절종괴형（반혹불반린근도관확장）；③은닉성유두상류，기부견확장적도관，미견명학실성결절。유선도관내유두상류적시간‐신호강도곡선여 ADC 치표현다양，기진단수결합형태학。결론 MRI 대병조형태급공능성상구유우세，가위림상학진유선도관내유두상류제공일정삼고개치。
Objective To analyze MRI features of intraductal papilloma of breast and the diagnostic value of MRI .Meth‐ods We retrospectively reviewed MRI features of 31 cases of intraductal papillomas confirmed by pathology ,including morphology ,signal intensity ,the type of TIC ,signal on DWI and ADC value .Results MRI revealed three patterns :the first type presented solid nodules in cystic dilatation of the ductal ,the second type showed a solid nodule mass with or without adjacent ductal dilatation ;and the third type was occult papilloma ,which only presented with distention of ducts and no solid nodule was observed .The TIC and and ADC value were various ,therefore ,diagnosis of the tumor should be combined with morphology .Conclusion MRI has advantage in showing morphology of lesion and functional imaging , which can provide some reference value for clinical diagnosis of intraductal papilloma .
