Pulse indicator continuous cadiac discharge technology in monitoring the volume%Cardiac index%Extravascular lung water index%Global end diastolic volume index
目的:探讨脉搏轮廓温度稀释连续心排血量监测技术( PICCO)监测指导下液体管理对肺水肿危重患者心排量指数( CI)、血管外肺水含量指数( EVLWI)、全心舒张末期容积指数( GEDVI)的影响。方法将80例肺水肿危重患者随机分为两组,各40例。两组均给予镇静、强心、扩血管等常规治疗。对照组行颈内静脉导管置管监测中心静脉压进行液体管理,观察组行PICCO监测血流动力学变化根据结果进行液体管理。观察两组管理前后CI、EVLWI、GEDVI各指标变化情况。结果两组管理后CI指标均较管理前升高( P <0.01),观察组升高幅度大于对照组( P <0.01);观察组管理后EVLWI指标较管理前下降( P <0.01),GEDVI指标较管理前升高( P <0.01)。结论通过PICCO监测能有效指导肺水肿危重患者早期液体管理,有助于肺水肿危重患者救治。

목적:탐토맥박륜곽온도희석련속심배혈량감측기술( PICCO)감측지도하액체관리대폐수종위중환자심배량지수( CI)、혈관외폐수함량지수( EVLWI)、전심서장말기용적지수( GEDVI)적영향。방법장80례폐수종위중환자수궤분위량조,각40례。량조균급여진정、강심、확혈관등상규치료。대조조행경내정맥도관치관감측중심정맥압진행액체관리,관찰조행PICCO감측혈류동역학변화근거결과진행액체관리。관찰량조관리전후CI、EVLWI、GEDVI각지표변화정황。결과량조관리후CI지표균교관리전승고( P <0.01),관찰조승고폭도대우대조조( P <0.01);관찰조관리후EVLWI지표교관리전하강( P <0.01),GEDVI지표교관리전승고( P <0.01)。결론통과PICCO감측능유효지도폐수종위중환자조기액체관리,유조우폐수종위중환자구치。
Objective To study the effect on the pulse indicator continuous cardiac discharge technology for monitoring the a -mount of ( PICCO) on pulmonary edema in critically ill patients with cardiac output index ( CI) ,extravascular lung water index ( EVL-WI) ,global end diastolic volume index ( GEDVI) .Methods Eighty critically ill patients with pulmonary were randomly divided into two groups with 40 cases in each group and were given sedative ,cardiac,vascular dilation and other conventional treatments ,but the con-trol group underwent internal jugular vein catheter monitoring central venous pressure of fluid management and the observation group un -derwent PICCO to observe the changes of hemodynamic mornitoring of liquid management .The changes of indexes of CI ,EVLWI,GEDVI before and after the management of the two groups were observed .Results CI of the two groups after treatment was higher than that be-fore treatment ( P <0.01),with increase of the observation group being bigger than that in the control group ( P <0.01).EVLWI of the observation group after treatment was lower than that before treatment ( P <0.01),but GEDVI increased instead ( P <0.01).Conclu-sion Mornitoring PICCO can effectively guide early liquid inputs and clinical treatment of critically ill patients with pulmonary edema .
