目的：探讨黄葵胶囊联合血管紧张素转换酶抑制剂（ ACEI）及血管紧张素Ⅱ受体拮抗剂（ ARB）治疗早期糖尿病肾病（ DN）的临床效果。方法将60例早期DN患者随机分为两组，各30例。对照组给予依那普利片和厄贝沙坦片治疗，观察组在对照组治疗基础上给予黄葵胶囊。对比两组治疗前后尿白蛋白排泄率（ UAER ）、醛固酮（ ALD ）、血管紧张素II （AII）、糖化血红蛋白（HbA1c）、血肌酐、尿素氮、血糖及平均动脉压（MAP）的变化，并观察药物不良反应发生情况。结果两组治疗前UAER、ALD、AII、HbA1c、血肌酐、尿素氮、血糖及MAP比较，差异均无统计学意义（ P ＞0．05）；两组治疗后血糖均无明显变化（ P ＞0．05）；观察组治疗后UAER、ALD、AII、HbA1c、血肌酐、尿素氮和MAP均低于治疗前（ P ＜0．05）；对照组治疗后UAER、ALD、AII、尿素氮和MAP均低于治疗前（ P ＜0．05）；两组治疗后比较，观察组UAER、ALD、AII、HbA1c、血肌酐和尿素氮均低于对照组（ P ＜0．05或P ＜0．01）。两组患者治疗期间不良反应发生率比较，差异无统计学意义（ P ＞0．05）。结论黄葵胶囊联合ACEI及ARB治疗早期DN效果显著，且使用安全。
목적：탐토황규효낭연합혈관긴장소전환매억제제（ ACEI）급혈관긴장소Ⅱ수체길항제（ ARB）치료조기당뇨병신병（ DN）적림상효과。방법장60례조기DN환자수궤분위량조，각30례。대조조급여의나보리편화액패사탄편치료，관찰조재대조조치료기출상급여황규효낭。대비량조치료전후뇨백단백배설솔（ UAER ）、철고동（ ALD ）、혈관긴장소II （AII）、당화혈홍단백（HbA1c）、혈기항、뇨소담、혈당급평균동맥압（MAP）적변화，병관찰약물불량반응발생정황。결과량조치료전UAER、ALD、AII、HbA1c、혈기항、뇨소담、혈당급MAP비교，차이균무통계학의의（ P ＞0．05）；량조치료후혈당균무명현변화（ P ＞0．05）；관찰조치료후UAER、ALD、AII、HbA1c、혈기항、뇨소담화MAP균저우치료전（ P ＜0．05）；대조조치료후UAER、ALD、AII、뇨소담화MAP균저우치료전（ P ＜0．05）；량조치료후비교，관찰조UAER、ALD、AII、HbA1c、혈기항화뇨소담균저우대조조（ P ＜0．05혹P ＜0．01）。량조환자치료기간불량반응발생솔비교，차이무통계학의의（ P ＞0．05）。결론황규효낭연합ACEI급ARB치료조기DN효과현저，차사용안전。
Objective To explore the clinical effect for the treatment of early diabetic nephropathy (DN)with okra capsule, combined with ACEI and ARB .Methods Sixty patients with DN were divided into two groups ,30 cases in each group .The control group was given enalapril and irbesartan tablets treatment ,the experimental group was given okra capsule on the basis of treatment of the control group.Two groups were observed for urinary albumin excretion rate (UAER) and aldosterone (ALD) and angiotensin II(AII), blood glucose,blood urea nitrogen,creatinine,mean arterial pressure(MAP)before and after treatment and adverse reaction was ob-served.Results Before treatment,there was no significant difference in UAER ,ALD,AII,urea nitrogen and MAP between the two groups( P >0.05),but after treatment UAER,ALD,AII,HbA1c,serum creatinine,urea nitrogen and MAP of the observation group were lower than that before treatment ( P <0.05).UAER,ALD,AII,HbA1c,serum creatinine and urea nitrogen and MAP of the con-trol group after treatment were lower than that before treatment ( P <0.05)and UAER,ALD,AII,HbA1c,serum creatinine and urea ni-trogen of the observation group after treatment were lower than that of the control group ( P <0.05 or P <0.01) and the adverse reac-tion rates of the two groups during treatment had no significant differences ( P >0.05).Conclusion Okra capsule combined with ACEI and ARB for the treatment of patients with DN is effective and safe .
