目的:探讨超重和正常体质量青少年在递增负荷运动实验中血流动力学的变化。方法87名超重青少年分为女生超重组(GO,n=20),女生正常体质量组(GN,n=21),男生超重组(BO,n=25),男生正常体质量组(BN, n=21)。分别测定受试者的身体形态学参数,安静时以及递增负荷至力竭时的血流动力学参数包括安静收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、心率(HR)及最大SBP、DBP、MAP和HR。结果 BO组安静SBP高于BN组,差异有统计学意义(P<0.01)。递增负荷运动中,GO组最大SBP高于GN组、最大摄氧量(VO2max)低于GN组,差异均有统计学意义(P均<0.05);BO组最大SBP高于BN组、VO2max低于BN组,差异有统计学意义(P均<0.01)。将身高、体质量、BMI和VO2max作为混淆因子校正后,GO组和GN组间SBP的差异无统计学意义(P=0.105);BO组和BN组间安静SBP、最大SBP的差异仍有统计学意义(P=0.021、P=0.016))。结论超重青少年安静时和递增负荷运动中的动脉血压明显高于体质量正常者。超重对男性青少年的血流动力学参数的影响可能比女生更明显。

Objective To explore hemodynamics response to the graded exercise test in overweight and normal weight teenagers. Methods A total of 87 overweight teenagers were enrolled and divided into girl overweight group (GO, n=20), girl normal weight group (GN, n=21), boy overweigh group (BO, n=25) and boy normal weight group (BN, n=21). The following variables were measured:anthropometrics (weight, height and BMI), hemodynamic variables at rest and during graded exercise test such as rest systolic blood pressure (SBP), diastolic blood pressure (DBP), mean aterial pressure (MAP) and heart rate (HR) as well as maximal SBP, DBP, MAP and HR. Results The rest SBP in BO group was higher than that in BN group (P<0.01). During graded exercise test, compared with GN group, maximal SBP were signiifcantly higher while maximal oxygen uptake (VO2max) were signiifcantly lower in GO group (P<0.05). Furthermore, compared with BN group, maximal SBP were signiifcant-ly higher while VO2max were signiifcantly lower in BO group (P<0.01). When height, weight, BMI, and VO2max were adjusted as confounding factors, the difference in SBP was not signiifcant between GO and GN groups (P=0.105) while the differences in rest SBP and maximal SBP were still signiifcant between BO and BN groups (P=0.021, P=0.016). Conclusions Arterial blood pressure was higher in overweight teenagers than that in normal weight teenagers both at rest and during graded exercise test. Inlfuence of overweight on hemodynamic parameters was more obvious in male teenagers in comparison with female teenagers.
