目的:观察多模式镇痛方案对全膝关节表面置换术后的疼痛控制及早期功能康复效果。方法自2013年3月~2014年3月共60例单侧人工全膝关节表面置换患者随机分为研究组和对照组,每组30例。研究组患者采用多模式的镇痛方案,对照组患者手术结束后给予硬膜外自控镇痛方案。分别记录术后不同时间静息、持续被动功能训练时的视觉模拟疼痛评分( VAS)和术后膝关节主动活动度。结果60例患者全部进入结果分析。两组患者术后不同时间静息、持续被动功能训练时VAS结果比较差异非常显著( P<0.01);术后膝关节主动活动度结果比较差异亦有统计学意义(P<0.01)。结论多模式镇痛方案对膝关节置换术后的疼痛控制效果满意,且能有效促进膝关节置换术后的早期功能康复训练。

목적:관찰다모식진통방안대전슬관절표면치환술후적동통공제급조기공능강복효과。방법자2013년3월~2014년3월공60례단측인공전슬관절표면치환환자수궤분위연구조화대조조,매조30례。연구조환자채용다모식적진통방안,대조조환자수술결속후급여경막외자공진통방안。분별기록술후불동시간정식、지속피동공능훈련시적시각모의동통평분( VAS)화술후슬관절주동활동도。결과60례환자전부진입결과분석。량조환자술후불동시간정식、지속피동공능훈련시VAS결과비교차이비상현저( P<0.01);술후슬관절주동활동도결과비교차이역유통계학의의(P<0.01)。결론다모식진통방안대슬관절치환술후적동통공제효과만의,차능유효촉진슬관절치환술후적조기공능강복훈련。
[ ABSTRACT] Objective To study the effect of multimodal analgesia on postoperative pain controlling and ear-ly functional rehabilitation after total knee arthroplasty .Methods Atotal of 60 patients with unilateral total knee arthro-plasty from 2013 March to 2014 March were randomly divided into study group and control group ,30 cases in each group.Payients in study group were treated with multiple modes while patients in control group were given epidural con -trolled analgesia after operation .Vision( VAS) pain scores at different time resting ,passive functional training after oper-ation and the postoperative active range of motion were recorded .Results All 60 patients were involved in the result a-nalysis.The VAS results after different time resting and passive functional training of the two groups were statistically sig -nificant(P<0.01),the postoperative active range of motion between the two groups was also statistically different (P<0.01).Conclusion The effect of multimodal analgesia on pain controling after knee arthroplasty was satisfactory ,fur-thermore ,it can effectively promote the early functional rehabilitation after knee replacement .
