目的：通过比较悬吊式免气腹与气腹腹腔镜胃癌根治术的差异，探讨悬吊式免气腹腹腔镜下胃癌根治性的可行性及应用前景。方法选取潍坊市人民医院自2012年10月～2014年5月施行的悬吊式免气腹腹腔镜下胃癌根治性切除术31例，其中根治性远端胃切除术15例，根治性近端胃切除术9例，根治性全胃切除术7例，与随机选取同期完成的气腹腹腔镜胃癌根治术（相同分期）30例资料作回顾性分析。结果61例患者的手术均获成功，无中转开腹及术中死亡的情况发生。免气腹组术后肛门首次排气时间、术后并发症发生率优于气腹组（ P＜0．05）。结论免气腹腹腔镜用于胃癌根治术安全可行，能够达到根治要求，兼具有气腹腹腔镜的优点，比之更有并发症少且术后康复快的优势，具有推广价值。
목적：통과비교현조식면기복여기복복강경위암근치술적차이，탐토현조식면기복복강경하위암근치성적가행성급응용전경。방법선취유방시인민의원자2012년10월～2014년5월시행적현조식면기복복강경하위암근치성절제술31례，기중근치성원단위절제술15례，근치성근단위절제술9례，근치성전위절제술7례，여수궤선취동기완성적기복복강경위암근치술（상동분기）30례자료작회고성분석。결과61례환자적수술균획성공，무중전개복급술중사망적정황발생。면기복조술후항문수차배기시간、술후병발증발생솔우우기복조（ P＜0．05）。결론면기복복강경용우위암근치술안전가행，능구체도근치요구，겸구유기복복강경적우점，비지경유병발증소차술후강복쾌적우세，구유추엄개치。
[ ABSTRACT] Objective To compare the differences between under suspended gasless laparoscopic surgery and pneumoperitoneum laparoscopic surgery in gastric cancer gasless laparoscopic gastrectomy and laparoscopic gastrectomy gas ,and to explore the feasibility and ap-plication prospects of suspended Gasless Laparoscopic Surgerygastric radical under gasless laparoscopy .Methods Thirty-one patients with suspended Gasless Laparoscopic Surgery of gastric cancer in Weifang People 's Hospital from October 2012 to May 2014 were collected ,among them,15 cases with radical distal gastrectomy ,9 cases with radical proximal gastrectomy and 7 cases with radical total gastrectomy .A retro-spective analysis between data above and another 30 cases with gasless laparoscopic gastric cancer (in the same stage) radical prostatectomy at the same term was made.Results All of the operations were successful without turning to open .61 cases of patients were successful,no conversion to open surgery cases and deaths occured;surgery time gasless group's first anal exhaust time and postoperative complication rate was lower than that of pneumoperitoneum group (P<0.05).Conclusion Gasless laparoscopic radical gastrectomy is safe and feasible ,be-sides,becauso of its achieved radical demands and also has the advantages of laparoscopic insufflationinsufflations ,it is worthy of populari-zing.
