目的:采用Meta分析的方法探讨肿瘤坏死因子-α(TNF-α)基因G-238A多态性与冠状动脉疾病(CAD)风险的相关性。方法检索PubMed、Embase和中国知网数据库,收集探讨TNF-α基因G-238A多态性与CAD风险的相关性的病例-对照研究,检索时限均从建库至2014年5月13日。由两名研究者独立选择筛选文献、提取资料后,使用Comprehensive Meta-Analysis软件进行Meta分析。结果最终10篇文献共11项病例-对照研究。Meta分析结果表明G-238A多态性与CAD风险相关性无统计学意义[A vs. G:OR=1.08,95%CI:0.89~1.30,I2=34.33%;AA vs. GG:OR=1.15,95%CI:0.59~2.25,I2=0%;GA vs. GG:OR=1.14,95%CI:0.88~1.48,I2=54.34%;AA vs.(GG+GA):OR=1.09,95%CI:0.56~2.14, I2=0%;(GA+AA) vs. GG:OR=1.11,95%CI:0.90~1.38,I2=41.92%]。敏感度分析显示研究结果稳定性较好。亚组分析同样得到了类似结果。未检测到发表偏倚。结论 TNF-α基因G-238A多态性与CAD易感性可能无相关性,建议开展高质量研究进一步论证。

Objective To investigate the correlation between G-238A polymorphism of tumor necrosis factor-α (TNF-α) and risk of coronary artery disease (CAD) by applying Meta-analysis. Methods The databases of PubMed, EMbase and CNKI were retrieved for collecting the case-control studies on correlation between G-238A polymorphism of TNF-α and CAD risk from database establishment time to May 13, 2014. After screening literature and extracting data by 2 independent researchers, the data was given a Meta-analysis by using Comprehensive Meta-Analysis software. Results There were finally 11 case-control studies included in 10 literature. The results of Meta-analysis showed that the correlation between G-238A polymorphism and CAD risk had no statistical significance [A vs. G:OR=1.08, 95%CI:0.89~1.30, I2=34.33%;AA vs. GG:OR=1.15, 95%CI: 0.59~2.25, I2=0%; GA vs. GG: OR=1.14, 95%CI: 0.88~1.48, I2=54.34%; AA vs. (GG+GA): OR=1.09, 95%CI:0.56~2.14, I2=0%;(GA+AA) vs. GG:OR=1.11, 95%CI:0.90~1.38, I2=41.92%]. The sensitivity analysis showed that all results were robust, and subgroup analysis had the similar results. No publication bias was detected. Conclusion G-238A polymorphism of TNF-αmaybe not correlated to CAD risk. It is suggested to conduct more higher-quality studies for further verification.
