目的:观察腹腔神经丛阻滞( NCPB)对肝部分切除术( PH)大鼠术后肾功能的影响。方法分别于雄性SD大鼠肝脏左、中叶根部结扎、切除,构建经典大鼠PH模型,然后随机分为对照组和NCPB组。NCPB组在术后12 h开始予0.5%利多卡因1 ml行双侧经皮NCPB,1次/d,连续7 d;对照组以0.9%生理盐水代替。分别于术后第1、3、7 d,NCPB后30 min测定大鼠肾功能、肾脏组织超微结构、肾血流灌注及肾脏总抗氧化能力( T-AOC)。结果 NCPB组血尿素氮( BUN)和肌酐( Cr)在PH术后第1、3 d显著低于对照组( p<0.05或p<0.01),第7 d两组的BUN和Cr差异无统计学意义;在术后第1、3和7 d, NCPB组肾血流量在各时间点均显著高于对照组( p<0.05或p<0.01);在术后第1、3和7 d,NCPB组的肾组织T-AOC各时间点均显著高于对照组( p<0.05或p<0.01);而术后肾组织超微结构的改变NCPB组明显轻于对照组,其恢复也早于对照组。结论 NCPB能够减轻PH术后大鼠的肾脏损害,可能与增加肾血流灌注,减轻肾脏组织的氧化应激反应有关。

목적:관찰복강신경총조체( NCPB)대간부분절제술( PH)대서술후신공능적영향。방법분별우웅성SD대서간장좌、중협근부결찰、절제,구건경전대서PH모형,연후수궤분위대조조화NCPB조。NCPB조재술후12 h개시여0.5%리다잡인1 ml행쌍측경피NCPB,1차/d,련속7 d;대조조이0.9%생리염수대체。분별우술후제1、3、7 d,NCPB후30 min측정대서신공능、신장조직초미결구、신혈류관주급신장총항양화능력( T-AOC)。결과 NCPB조혈뇨소담( BUN)화기항( Cr)재PH술후제1、3 d현저저우대조조( p<0.05혹p<0.01),제7 d량조적BUN화Cr차이무통계학의의;재술후제1、3화7 d, NCPB조신혈류량재각시간점균현저고우대조조( p<0.05혹p<0.01);재술후제1、3화7 d,NCPB조적신조직T-AOC각시간점균현저고우대조조( p<0.05혹p<0.01);이술후신조직초미결구적개변NCPB조명현경우대조조,기회복야조우대조조。결론 NCPB능구감경PH술후대서적신장손해,가능여증가신혈류관주,감경신장조직적양화응격반응유관。
Objective To observe the effect of neurolytic celiac plexus block( NCPB )on renal function of rats after partial hepatectomy( PH). Methods Ligation and resection were made in the left and middle liver of male SD rates in order to build classical PH model rats,which were then randomly divided into control group and NCPB group. 1 ml of 0. 5% lidocaine was administered to the rats in the NCPB group for bilateral percutaneous NCPB 12 hours later from the operation,one time per day,for seven successive days. 0. 9%normal saline was used in the control group,instead. Within 30 min after NCPB on the 1st day,the 3rd day,and the 7th day,the rat renal function,renal tissue ultrastructure,renal blood perfusion and total antioxidant capacity(T-AOC)were determined respectively. Resuits The postoperative blood urea nitrogen(BUN)and creatinine(Cr)on the 1st day and the 3rd day in the NCPB group were significantly lower than those in the control group(p<0. 05 or p<0. 01),and on the 7th day,BUN and Cr in the two groups had not statistically significant difference;the postoperative renal blood flows on the 1st day,the 3rd day,and the 7th day in the NCPB group were all higher than those in the control group(p<0. 05 or p<0. 01),while the changes of postoperative renal tissue ultrastructure in the NCPB group were significantly less than those in the control group,and the recovery was also earlier than that in the control group. Conciusion NCPB can reduce renal damage in rats after PH,which may be associated with renal blood perfusion increase and oxidative stress reduction of renal tissue.
