目的：比较噻托溴铵联用不同规格的沙美特罗替卡松对老年稳定期慢性阻塞性肺疾病（ COPD）的疗效。方法将128例老年稳定期COPD患者随机分为A组与B组，两组均给予基础治疗和噻托溴铵治疗，此外，A组加用50μg/250μg沙美特罗替卡松，B组加用50μg/500μg沙美特罗替卡松，两组疗程均为12个月。比较两组治疗后动脉血气、肺功能和运动功能指标及临床疗效的差异。结果治疗12个月后，两组的PaO2、PaCO2、FEV1、FVC、FEV1/FVC、6 min步行距离（6MWD）均比治疗前有显著改善（p<0.05），且B组这些指标的改善程度均优于A组（p<0.05），临床疗效也明显优于A组（p<0.05）。结论噻托溴铵联用50μg/500μg 沙美特罗替卡松治疗老年稳定期 COPD 疗效确切，综合效果优于噻托溴铵联用50μg/250μg沙美特罗替卡松，可在临床上推荐使用。
목적：비교새탁추안련용불동규격적사미특라체잡송대노년은정기만성조새성폐질병（ COPD）적료효。방법장128례노년은정기COPD환자수궤분위A조여B조，량조균급여기출치료화새탁추안치료，차외，A조가용50μg/250μg사미특라체잡송，B조가용50μg/500μg사미특라체잡송，량조료정균위12개월。비교량조치료후동맥혈기、폐공능화운동공능지표급림상료효적차이。결과치료12개월후，량조적PaO2、PaCO2、FEV1、FVC、FEV1/FVC、6 min보행거리（6MWD）균비치료전유현저개선（p<0.05），차B조저사지표적개선정도균우우A조（p<0.05），림상료효야명현우우A조（p<0.05）。결론새탁추안련용50μg/500μg 사미특라체잡송치료노년은정기 COPD 료효학절，종합효과우우새탁추안련용50μg/250μg사미특라체잡송，가재림상상추천사용。
Objective To compare the curative effect of tiotropium bromide combined with salmeterol fluticasone of different specifications in treatment of chronic obstructive pulmonary disease( COPD)at stable stage in senile patients. Methods Total 128 senile patients with COPD were randomly divided into group A and group B,and both groups were provided with basic treatment of tiotropium bromide,on the basic treatment,the patients in group A were provided with 50 μg/250 μg salmeterol fluticasone,and group B with 50 μg/500 μg salmeterol fluticasone. The courses of treatment in two groups were 12 months. And then,the differences in arterial blood gas,lung function and exercise function and the clinical effect after the treatment were compared between two groups. Resuits After the treatment for 12 months,the PaO2,PaCO2,FEV1,FVC,FEV1/FVC,and 6-min walking distance(6MWD)in two groups were improved significantly than those before the treatment(p<0. 05);the improvement of these indicators in group B was better than those in group A (p<0. 05),and the clinical curative effect in group B was obviously better than that in group A(p<0. 05). Conciusion Tiotropium bromide combined with 50 μg/500 μg salmeterol fluticasone has exact curative effect on senile COPD at stable stage,and the comprehensive effect is better than that of tiotropium bromide combined with 50 μg/250 μg salmeterol fluticasone,so it can be recommended in clinical use.
