目的：观察封闭负压引流（ vacuum-assisted closure ，VAC）对兔放射性溃疡创面组织愈合的影响。方法制作兔放射性溃疡模型，设空白对照组和实验组（对照组创面采用常规包扎治疗处理，实验组创面则采用VAC处理），在致伤前、致伤后3、7、14 d及创面愈合后5个不同时相点进行各项研究指标的观测。结果实验组较对照组创面肿胀及分泌物得到明显控制，创面坏死组织的清除与肉芽组织的生长明显加快；平均愈合时间明显缩短[（16．3±2．6）d比（20．4±3．7）d）]（P＜0．05）；损伤后3、7和14 d创面组织MMP-1和MMP-2 mRNA的表达明显减少，差异有统计学意义（P＜0．05），TIMP-1和TIMP-2 mRNA的表达明显增加，差异有统计学意义（P＜0．05），血清bFGF和VEGF的表达明显升高，差异有统计学意义（P＜0．05）。结论 VAC对兔放射性溃疡创面组织的愈合起到积极的促进作用。
목적：관찰봉폐부압인류（ vacuum-assisted closure ，VAC）대토방사성궤양창면조직유합적영향。방법제작토방사성궤양모형，설공백대조조화실험조（대조조창면채용상규포찰치료처리，실험조창면칙채용VAC처리），재치상전、치상후3、7、14 d급창면유합후5개불동시상점진행각항연구지표적관측。결과실험조교대조조창면종창급분비물득도명현공제，창면배사조직적청제여육아조직적생장명현가쾌；평균유합시간명현축단[（16．3±2．6）d비（20．4±3．7）d）]（P＜0．05）；손상후3、7화14 d창면조직MMP-1화MMP-2 mRNA적표체명현감소，차이유통계학의의（P＜0．05），TIMP-1화TIMP-2 mRNA적표체명현증가，차이유통계학의의（P＜0．05），혈청bFGF화VEGF적표체명현승고，차이유통계학의의（P＜0．05）。결론 VAC대토방사성궤양창면조직적유합기도적겁적촉진작용。
Objective To observe the effects of vacuum-assisted closure ( VAC) on the healing of the radioactive ulcer in rabbits.Methods The rabbit radioactive ulcer model was established , and the animals were divided into 2 groups:the control group treated with conventional wound dressing and the experimental group treated with VAC .Observations were made on various indicators at 5 different time points:i.e.before surgery, 3, 7 and 14 days after surgery, and after wound healing.Results Wound swelling and se-cretions for the animals of the experimental group were significantly improved , when they were compared with those of the control group . Removal of necrotic tissue and growth of granulation tissue were significantly accelerated .Average healing time was significantly short-ened [(16.3 ±2.6)d比(20.4 ±3.7)d)] (P<0.05).The expression levels of MMP-1 and MMP-2 mRNA in the wound tissue were significantly decreased, 3, 7 and 14 days after surgery (P<0.05), while the expression levels of TIMP-1 and TIMP-2 mRNA were sig-nificantly increased (P <0.05).The expressions of bFGF and VEGF in the serum were increased considerably (P <0.05). Conclusion Vacuum-assisted closure displayed a positive effect on the healing of radioactive ulcer of rabbits .
