目的:探讨荧光标记的嗜水气单胞菌溶素变异体(FLAER)检测在阵发性睡眠性血红蛋白尿症(PNH)诊断中的意义,并与传统的CD55、CD59测定进行比较。方法采用流式细胞术检测10例PNH、37例非PNH贫血患者外周血FLAER、CD55和CD59抗原的表达,对其结果进行比较。结果与非 PNH 贫血组相比,PNH 患者外周血 CD55、CD59缺失率和FLAER阴性率均明显增高(P<0.05)。10例PNH患者外周血粒细胞FLAER阴性率达(82.52±25.24)%,单核细胞FLAER阴性率达(79.84±19.86)%;与CD55、CD59的阴性率相比,PNH克隆数显著升高(P<0.05)。结论利用流式细胞术进行FLAER分析对PNH的诊断比CD55、CD59检测更加特异、敏感、直观,FLAER分析更能反映PNH克隆情况。

Objective To investigate the significance of FLAER detection in the diagnosis of paroxysmal nocturnal hemoglobinuria (PNH). Method Expressions of CD55 and CD59 antigens and FLAER in the cell membranes of peripheral blood were detected in 10 cases with PNH and a control group of 37 patients with malnutritional anemia by flow cytometry. Results The negative rates of CD55, CD59 and FLAER in the PNH patients were significantly higher than those in patients in the control group (P<0.05). In 10 cases of PNH patients, the negative rates of FLAER in the granulocyte population and monocyte population was (82.52±25.24)% and (79.84±19.86)% respectively. Compared with the negative rates of CD55 and CD59, a higher percentage of PNH clones was detected by the FLAER-based PNH panel (P<0.05). Conclusion FLAER detection with flow cytometry can im-prove the current methods used in diagnosis of PNH by specifically identifying PNH clones in the granulocyte and monocyte popu-lation.
