目的：探讨经颌后切口入路治疗髁突中低位骨折的临床效果。方法：对18例经颌后入路行髁突中低位骨折切开复位内固定术进行回顾分析，评价患者张口度、开口型、咬合关系、面部表情肌功能、涎瘘等临床指标。结果：18例患者术后咬合关系、张口度、开口型均可恢复正常，无面瘫、涎瘘病例出现。术后3个月复查CT片显示髁突解剖复位，骨折断端无成角或裂开。结论：经颌后切口入路治疗髁突中低位骨折，能充分保护面神经分支，最大程度减轻对腮腺腺体创伤，且能达到髁突解剖复位和坚固内固定的治疗效果。
목적：탐토경합후절구입로치료과돌중저위골절적림상효과。방법：대18례경합후입로행과돌중저위골절절개복위내고정술진행회고분석，평개환자장구도、개구형、교합관계、면부표정기공능、연루등림상지표。결과：18례환자술후교합관계、장구도、개구형균가회복정상，무면탄、연루병례출현。술후3개월복사CT편현시과돌해부복위，골절단단무성각혹렬개。결론：경합후절구입로치료과돌중저위골절，능충분보호면신경분지，최대정도감경대시선선체창상，차능체도과돌해부복위화견고내고정적치료효과。
Objective: To review the clinical effects of a modified posterior mandibular incision approach in the reduction of condylar fractures. Methods: 18 cases were retrospectively reviewed. The occlusal, relationship, degree of mouth open-ing, facial muscle function, salivary fistula and other clinical indicators were evaluated. Results: All the patients restored to normal occlusion and normal degree of mouth opening. No facial paralysis or salivary fistulas was revealed. Three months postoperative CT images showed all patients achieved anatomical repositioning of fractured condyle. Conclusion:Modified posterior mandible incision approach demonstrates benefits in the treatment of condylar fractures.
