目的:分析慢性阻塞性肺疾病( COPD)并发肺部感染患者病原菌分布情况及其耐药性。方法选取2012—2013年在英德市人民医院接受治疗的COPD并发肺部感染患者350例,共收集到病原菌256株。观察病原菌种类及主要病原菌耐药性。结果 COPD并发肺部感染患者的主要病原菌为革兰阴性杆菌,共188株(73.43%),其中前4种为肺炎克雷伯菌64株(25.00%)、铜绿假单胞菌42(16.40%)、鲍曼醋酸钙不动杆菌34株(13.28%)、大肠埃希菌22株(8.59%);其次为革兰阳性杆菌56株(21.87%)、真菌12株(4.70%)。4种主要革兰阴性感菌对抗菌药物均表现出不同程度的耐药,其中对环丙沙星、头孢噻肟、左氧氟沙星的耐药率较高,对丁胺卡那霉素、亚胺培南、美罗培南、头孢哌酮/舒巴坦的耐药率较低。结论 COPD并发肺部感染患者病原菌中革兰阴性杆菌所占比例较大,且其耐药率较高,需要引起重视。

목적:분석만성조새성폐질병( COPD)병발폐부감염환자병원균분포정황급기내약성。방법선취2012—2013년재영덕시인민의원접수치료적COPD병발폐부감염환자350례,공수집도병원균256주。관찰병원균충류급주요병원균내약성。결과 COPD병발폐부감염환자적주요병원균위혁란음성간균,공188주(73.43%),기중전4충위폐염극뢰백균64주(25.00%)、동록가단포균42(16.40%)、포만작산개불동간균34주(13.28%)、대장애희균22주(8.59%);기차위혁란양성간균56주(21.87%)、진균12주(4.70%)。4충주요혁란음성감균대항균약물균표현출불동정도적내약,기중대배병사성、두포새우、좌양불사성적내약솔교고,대정알잡나매소、아알배남、미라배남、두포고동/서파탄적내약솔교저。결론 COPD병발폐부감염환자병원균중혁란음성간균소점비례교대,차기내약솔교고,수요인기중시。
Objective To analyze the drug resistance of pathogenic bacteria of COPD patients complicated with pulmonary infection. Methods A total of 350 COPD patients complicated with pulmonary infection were selected in the People's Hospital of Yingde from 2012 to 2013,and a total of 256 strains of pathogenic bacteria were collected. Pathogenic bacteria kinds and drug resistance of main pathogenic bacteria were observed. Results The main pathogenic bacteria was gram-negative bacilli(188 strains,73. 43%),including Klebsiella pneumoniae(64 strains,25. 00%),Pseudomonas aeruginosa(42 strains,16. 40%),Bauman Acinetobacter calcoaceticus(34 strains,13. 28%),Escherichia coli(22 strains,8. 59%);gram-positive bacilli was the second pathogenic bacteria(56 strains,21. 87%),and fungus was the third(12 strains, 4. 70%). Klebsiella pneumoniae,Pseudomonas aeruginosa,Bauman Acinetobacter calcoaceticus and Escherichia coli showed high resistance to ciprofloxacin,cefotaxime,levofloxacin,low resistance to amikacin,imipenem,meropenem,cefoperazone/sulbactam. Conclusion Gram-negative bacilli is the main pathogenic bacteria of COPD patients complicated with pulmonary infection,and its drug resistance rate is high,which should be pay more attention to.
