目的:探讨内科胸腔镜在提高胸腔积液诊断率中的临床应用价值。方法回顾性分析275例胸腔积液患者的相关临床资料,评估胸腔积液细胞学以及内科胸腔镜检查对胸腔积液的诊断率。结果内科胸腔镜对良性胸腔积液的诊断灵敏感(80.00%)与胸水细胞学检查的灵敏度(85.00%)比较差异无统计学意义(χ2=1.813,P =0.178)。两种方法对胸腔积液诊断的特异度均为100%;内科胸腔镜检查对恶性胸腔积液诊断的灵敏度(86.10%)显著高于胸水细胞学检查(54.01%)(χ2=45.883,P <0.001)。结论内科胸腔镜能明显提高胸腔积液的诊断率,尤其是对于恶性胸腔积液,具有较高的临床应用价值。

Objective To investigate the clinical value of medical thoracoscopy in improving the diagnostic accuracy for pleural effusion. Methods A retrospective analysis was performed on the clinical records of 275 patients with pleural effusion,and assessed the diagnostic accuracy of pleural effusion with cytology and medi-cal thoracoscopy. Results Medical thoracoscopy had a sensitivity of 80.00% in diagnosis of benign pleural effusion,compared to pleural fluid cytology of 85.00%,there was no statistically significant difference (χ2 =1.813,P =0.178).Both methods had specificity of 100% in diagnosis of pleural effusion.In diagnosis of ma-lignant pleural effusion,medical thoracoscopy had higher sensitivity of 86.10%,compared to pleural fluid cy-tology of 54.01%,there was statistically significant difference(χ2 =45.883,P <0.001). Conclusion Medi-cal thoracoscopy can significantly improve the diagnosis of pleural effusion,especially in malignant pleural effu-sion;it has a high clinical value.
