目的:探讨新生儿高间接胆红素(UCB)的变化对神经元特异性烯醇化酶(NSE)的影响及所对应的 NSE 值对新生儿产生的临床影响。方法选取我院2011年10月~2014年10月儿科病房收治的61例高 UCB 患儿作为高间胆组,58例足月新生儿作为对照组,测定两组新生儿的 UCB 与 NSE 值,并在新生儿出生7 d 后进行新生儿神经行为测定(NB-NA)评分。结果高间胆组 UCB、NSE 水平均较对照组高,差异均有统计学意义(P 均<0.001);高间胆组 NBNA 评分显著低于对照组,差异均有统计学意义(P <0.001)。经直线相关分析 NSE 含量与 NBNA 评分呈显著负相关(r =-0.881,P <0.001)。结论高 UCB 可以引起新生儿的脑损伤,NSE 值可以作为指标来预测高 UCB 对脑损伤的临床影响。

목적:탐토신생인고간접담홍소(UCB)적변화대신경원특이성희순화매(NSE)적영향급소대응적 NSE 치대신생인산생적림상영향。방법선취아원2011년10월~2014년10월인과병방수치적61례고 UCB 환인작위고간담조,58례족월신생인작위대조조,측정량조신생인적 UCB 여 NSE 치,병재신생인출생7 d 후진행신생인신경행위측정(NB-NA)평분。결과고간담조 UCB、NSE 수평균교대조조고,차이균유통계학의의(P 균<0.001);고간담조 NBNA 평분현저저우대조조,차이균유통계학의의(P <0.001)。경직선상관분석 NSE 함량여 NBNA 평분정현저부상관(r =-0.881,P <0.001)。결론고 UCB 가이인기신생인적뇌손상,NSE 치가이작위지표래예측고 UCB 대뇌손상적림상영향。
Objective To explore the impacts of neonatal high unconjugated bilirubin (UCB)changes on neuron specific enolization enzyme (NSE)and the clinical influence of the corresponding NSE value on neo-nates. Methods Sixty-one children with high UCB cared by pediatrics department of our hospital between October 2011 and October 2011 of 61 were chosen as the high UCB group,58 cases of full-term newborn as control group.Determined the UCB and NSE values of the two groups,7 days after birth the neonatal behav-ioral neurological assessment (NBNA)was performed for the two groups. Results High UCB group had higher UCB and NSE levels,compared to the control group,there were statistically significant differences (P<0.001),High UCB group had lower NBNA scores than the control group,the comparison between the two groups yielded statistically significant difference (P < 0.001).The linear correlation analysis revealed that NSE content had negative correlation with NBNA scores (r =-0.881,P <0.001). Conclusion High indi-rect bilirubin can cause neonatal brain injury,NSE values can be used as a marker to predict the clinical effects of high indirect bilirubin on brain damage.
