目的:探讨b值为150 s/mm2和800 s/mm2时DWI对肝左叶小病灶检出能力。方法:对35例肝脏左叶小病灶患者(共49个病灶,其中肝细胞癌13个,肝转移瘤16个,肝血管瘤11个,肝囊肿9个)分别行b=150 s/mm2和800 s/mm2的DWI,比较不同b值下DWI探测小病灶的能力。分别重建施加不同弥散梯度方向时对应的DWI,分析心脏运动对哪个弥散梯度方向的图像影响最重。结果:b=150 s/mm2发现全部49个小病灶,b=800 s/mm2仅发现17个,可见随着b值升高,DWI探测肝左叶小病灶的能力下降。对于肝左叶而言,心脏运动主要影响上下方向的弥散梯度图像,导致SNR下降,伪影增加。结论:b=150 s/mm2时DWI有助于减少心脏运动导致的肝左叶信号强度下降,提高小病灶检出率。

Objective:This study was aimed to investigate the DWI (b value was selected 150 and 800 s/mm2 respectively) de-tective ability of small lesions,which located in the left liver lobe. Methods:35 patients who had small lesions in the left lobe were imaged by DWI. The detective ability for small lesion was compared by different b-value DWI. And the different diffusion direction imaging of DWI were reconstructed,the most important influence of cardiac motion to DWI was analysed. Results:The all of 49 lesions were detected by DWI of b=150 s/mm2 in 35 patients,on the contrary,17 lesions by DWI of b=800 s/mm2. The higher b-value was used,the less lesions were detected. The cardiac motion mainly influenced the S/I diffusion direction,it led to artifacts and the reduction of the signal to noise ratio. Conclusion:The lower b-value DWI could be used to detect the small lesions in the left lobe.
