目的:探讨超声对腮腺肿瘤的诊断与鉴别诊断价值。方法:回顾性分析经手术病理证实的173例腮腺肿瘤的声像图资料。结果:173例腮腺肿瘤中,良性159例,超声诊断符合率71.1%(113/159),恶性14例,超声诊断符合率71.4%(10/14)。良性病变常表现为形态规则边界清楚的肿块,以混合瘤居多,其次为腺淋巴瘤;恶性病变常表现为形态不规则,边界不清晰的肿块,以黏液表皮样癌居多。良恶性肿瘤在形态、边界、包膜、有无液化(囊实混合)、钙化及CDFI等表现方面差异有统计学意义(P<0.05)。结论:高频彩超是腮腺良恶性肿瘤鉴别诊断的重要方法。

목적:탐토초성대시선종류적진단여감별진단개치。방법:회고성분석경수술병리증실적173례시선종류적성상도자료。결과:173례시선종류중,량성159례,초성진단부합솔71.1%(113/159),악성14례,초성진단부합솔71.4%(10/14)。량성병변상표현위형태규칙변계청초적종괴,이혼합류거다,기차위선림파류;악성병변상표현위형태불규칙,변계불청석적종괴,이점액표피양암거다。량악성종류재형태、변계、포막、유무액화(낭실혼합)、개화급CDFI등표현방면차이유통계학의의(P<0.05)。결론:고빈채초시시선량악성종류감별진단적중요방법。
Objective:To assess the value of ultrasonography in diagnosis and differential diagnosis of parotid tumors. Methods:A retrospective analysis was done for data of ultrasonograms of 173 patients with parotid tumors confirmed by surgery and pathol-ogy. Results:Of 173 cases,there were 159 benign tumors,the conform rate was 71.1% (113/159);there were 14 malignant tum-ors,the conform rate was 71.4% (10/14). Benign tumors demonstrated regular shape and clear margin. Among the 159 benign tu-mors pleomorphic adenomas were most common,followed by adenolymphoma. While the malignant cases were showed as irregular masses with unclear margin. The most common malignant tumors were mucoepidermoid carcinoma. Our study indicated that be-tween benign and malignant tumor,the difference of the sample rate in shape,border,envelope,with liquefaction (complex cystic and solid) or calcification and color Doppler flow imaging of benign and malignant tumor were statistically significant (P<0.05). Conclusions:The high frequency color Doppler imaging is an important method in judging the characteristics of parotid tumors. But it is difficult to totally coincide with pathological diagnosis to other parotid tumors.
