目的：探讨真菌性食管炎的内镜下诊断标准，相关发病因素及治疗的临床分析。方法对2004年10月至2013年10月经胃镜及细菌学涂片确诊的真菌性食管炎患者的临床资料进行分析。结果40例患者中，相关发病因素有：长期应用抗生素14例，长期应用免疫抑制剂8例，恶性肿瘤，化疗治疗7例，糖尿病7例。肝硬化 4例。本组男性28例，女性12例。男女比例约为2.3：1，男性明显多于女性，年龄22~80岁，平均年龄为53岁。其中26例有食管症状，胸骨后烧灼感 8例，吞咽疼痛 6例，反酸 4例，吞咽困难5例，胸骨后疼痛3例。其余14例均表现为上腹部疼痛不适。经服用氟康唑及铝镁加混悬液治疗后症状明显改善。2周后复查胃镜，内镜下食管粘膜糜烂及白色附着物基本消失，粘膜真菌涂片(-)。结论真菌性食管炎相关发病因素较多，但临床上以去除病因，再经抗真菌治疗，患者均能得到肯定疗效。

목적：탐토진균성식관염적내경하진단표준，상관발병인소급치료적림상분석。방법대2004년10월지2013년10월경위경급세균학도편학진적진균성식관염환자적림상자료진행분석。결과40례환자중，상관발병인소유：장기응용항생소14례，장기응용면역억제제8례，악성종류，화료치료7례，당뇨병7례。간경화 4례。본조남성28례，녀성12례。남녀비례약위2.3：1，남성명현다우녀성，년령22~80세，평균년령위53세。기중26례유식관증상，흉골후소작감 8례，탄인동통 6례，반산 4례，탄인곤난5례，흉골후동통3례。기여14례균표현위상복부동통불괄。경복용불강서급려미가혼현액치료후증상명현개선。2주후복사위경，내경하식관점막미란급백색부착물기본소실，점막진균도편(-)。결론진균성식관염상관발병인소교다，단림상상이거제병인，재경항진균치료，환자균능득도긍정료효。
Objective to study the fungal esophagitis endoscopic diagnosis standard, related risk factors and treatment of clinical analysis.Methods in october 2004 to 2013, 10 menstrual gastroscopy and bacteriology smear diagnosed fungal esophagitis patients clinical data for analysis.Results of 40 cases, related risk factors are: 14 cases of long-term use of antibiotics, long-term application of immunosuppressant 8 cases, malignant tumor, chemotherapy in the treatment of 7 cases, 7 cases of diabetes. cirrhosis of the liver in 4 cases. this group of male 28 cases, 12 cases were women. the sex ratio of 2.3:1, more men than women, apparently, aged 22 to 80 years old, with an average age of 53. 26 cases with esophageal symptoms, retrosternal burning sensation in the 8 cases, 6 cases swallowing pain, anti acid 4 cases, 5 cases swallowing difficulties, retrosternal pain in 3 cases. the rest of the 14 cases were presented with epigastric pain discomfort. By taking fluconazole and aluminum magnesium and mixed suspension symptoms improved after treatment. after 2 weeks review of gastroscope, endoscopic esophageal mucosal erosion and attachments to basic white disappeared, mucosal fungal smear (-).Conclusion fungal esophagitis associated risk factors is more, but to remove the etiology, clinical and antifungal treatment, patients can get positive results.
