目的：探讨老年原发性高血压患者餐后血糖水平与心率变异性间的相关性。方法：2012年3月-2014年8月体检老年原发性高血压患者204例，分为餐后血糖正常组、餐后血糖升高组、糖尿病组。所有受试者均进行24 h动态心电图测定。结果：餐后血糖正常组、餐后血糖升高组和糖尿病组 SDNN 中位数分别为112.36、86.68和68.43 ms， SDANN 分别为96.47、74.63和63.51 ms，rMSSD 分别为38.02、26.33和20.18 ms，两两比较，差异均有统计学意义(P＜0.05)。LF分别为773.41、552.23和469.92 ms2，HF分别为402.60、334.55和340.52 ms2，LF/HF分别为1.92、1.65和1.38，餐后血糖正常组显著高于餐后血糖升高组和糖尿病组(P＜0.05)。餐后血糖水平与 SDNN、SDANN、rMSSD、LF和LF/HF呈显著负相关，与VLF呈正相关(P＜0.05)。结论：老年原发性高血压患者餐后血糖水平升高可能是心脏自主神经功能紊乱的重要原因。
목적：탐토노년원발성고혈압환자찬후혈당수평여심솔변이성간적상관성。방법：2012년3월-2014년8월체검노년원발성고혈압환자204례，분위찬후혈당정상조、찬후혈당승고조、당뇨병조。소유수시자균진행24 h동태심전도측정。결과：찬후혈당정상조、찬후혈당승고조화당뇨병조 SDNN 중위수분별위112.36、86.68화68.43 ms， SDANN 분별위96.47、74.63화63.51 ms，rMSSD 분별위38.02、26.33화20.18 ms，량량비교，차이균유통계학의의(P＜0.05)。LF분별위773.41、552.23화469.92 ms2，HF분별위402.60、334.55화340.52 ms2，LF/HF분별위1.92、1.65화1.38，찬후혈당정상조현저고우찬후혈당승고조화당뇨병조(P＜0.05)。찬후혈당수평여 SDNN、SDANN、rMSSD、LF화LF/HF정현저부상관，여VLF정정상관(P＜0.05)。결론：노년원발성고혈압환자찬후혈당수평승고가능시심장자주신경공능문란적중요원인。
Objectives:To investigate the correlation between postprandial plasma glucose and heart rate variability in elderly essential hypertensive patients.Methods:204 cases of elderly essential hypertensive patients from March 2012 TO August 2014 were divided into the normal postprandial plasma glucose group,high postprandial plasma glucose group and the diabetes group. All participants underwent 24 h ambulatory electrocardiogram.Results:The normal standard deviation of RR interval(SDNN) in the normal postprandial plasma glucose group,high postprandial plasma glucose group and the diabetes group was 112.36,86.68 and 68.43 ms;the SDANN was 96.47,74.63 and 63.51 ms,and the rMSSD was 38.02,26.33 and 20.18 ms;there were significant differences among three groups(P<0.05).And low frequency (LF) was 773.41,552.23 and 469.92 ms2,high frequency (HF) was 402.60,334.55 and 340.52 ms2,LF/HF rate was 1.92,1.65 and 1.38 respectively.LF,HF and LF/HF rate in high postprandial plasma glucose group were significantly lower than that in control group(P<0.05).The level of postprandial plasma glucose was significantly negatively related to SDNN,SDANN,rMSSD,LF and LF/HF rate,and positively related to VLF(P<0.05).Conclusion:High level of postprandial plasma glucose maybe the most important risk factor of autonomic cardiovascular dysfunction for elderly essential hypertensive patients.
