目的:分析比较普通 X 线片和64层螺旋 CT 三维容积重建在肋骨骨折诊断中的应用价值。方法:2012年6月-2014年8月收治肋骨骨折患者89例,分别进行普通X线片和64层螺旋CT三维容积重建诊断,观察对比两种检查图像对肋骨骨折的诊断敏感性。结果:89例肋骨骨折患者中,CT轴位扫描诊断全部肋骨骨折241处,以CT轴位扫描诊断为标准。普通 X 线片检查诊断骨折54例142处,敏感性58.92%;64层螺旋 CT 三维容积重建诊断骨折83例236处,敏感性97.93%,两者阳性率之间差异具有统计学意义(P<0.01)。普通X线与64层螺旋CT对膈上肋骨骨折、膈下肋骨骨折、胸肋关节处及细微骨折检查的阳性率差异具有统计学意义(P<0.01)。结论:普通X线片只能初步检查肋骨骨折的发生,漏诊率较高,与普通X线相比,64层螺旋CT三维容积重建检查诊断精确,可作为诊断肋骨骨折的最佳检查方法。

목적:분석비교보통 X 선편화64층라선 CT 삼유용적중건재륵골골절진단중적응용개치。방법:2012년6월-2014년8월수치륵골골절환자89례,분별진행보통X선편화64층라선CT삼유용적중건진단,관찰대비량충검사도상대륵골골절적진단민감성。결과:89례륵골골절환자중,CT축위소묘진단전부륵골골절241처,이CT축위소묘진단위표준。보통 X 선편검사진단골절54례142처,민감성58.92%;64층라선 CT 삼유용적중건진단골절83례236처,민감성97.93%,량자양성솔지간차이구유통계학의의(P<0.01)。보통X선여64층라선CT대격상륵골골절、격하륵골골절、흉륵관절처급세미골절검사적양성솔차이구유통계학의의(P<0.01)。결론:보통X선편지능초보검사륵골골절적발생,루진솔교고,여보통X선상비,64층라선CT삼유용적중건검사진단정학,가작위진단륵골골절적최가검사방법。
Objective:To analyze and compare the application values of ordinary X ray film and 64 layers spiral CT three-dimensional volume reconstruction for rib fracture.Methods:89 patients with rib fracture were selected from June 2012 to August 2014.They were respectively diagnosed by ordinary X ray film and 64 layers spiral CT three-dimensional volume reconstruction.The diagnostic sensitivitys of two check images for rib fracture were observed and compared.Results:In 89 patients with rib fracture,CT axial scanning diagnosed all rib fracture in 241,and CT axial scanning diagnosis was standard.The ordinary X ray film examination diagnosed 54 cases of fracture in 142,and the sensibility was 58.92%.64 layers spiral CT three-dimensional volume reconstruction diagnosed 83 cases of fracture in 236,and the sensibility was 97.93% .There was significant difference between the positive rate of the two,and the difference was statistically significant(P<0.01).The difference of the positive rates between ordinary X-ray and 64 layer spiral CT for rib fracture on diaphragm,subphrenic rib fracture,chest rib joints and tiny fractures was outstanding with statistical significance(P<0.01).Conclusion:The ordinary X ray only has a preliminary inspection of rib fracture,and its missed diagnosis is high.Compared with the ordinary X-ray, 64 layers spiral CT three-dimensional volume reconstruction diagnoses accurately,it is best inspection method in the diagnosis of rib fracture.
