目的:了解铜绿假单胞菌对亚胺培南的耐药机制。方法:选取临床分离的铜绿假单胞菌89株,采用纸片扩散法行药敏试验、紫外线吸收法行膜微孔蛋白oprD2基因测定、纸片协同试验测定金属β-内酰胺酶,同时采用Etest试验了解氰氯苯腙对亚胺培南最低抑菌浓度的影响。对所有试验结果进行统计分析。结果:耐药铜绿假单胞菌菌株oprD2含量明显低于敏感铜绿假单胞菌菌株(P<0.05);且有耐药铜绿假单胞菌菌株产生β-内酰胺酶;同时还有耐药铜绿假单胞菌菌株存在主动外排泵出机制。结论:oprD2含量降低、β-内酰胺酶产生、主动外排泵出机制是导致铜绿假单胞菌对亚胺培南耐药的主要作用机制。

목적:료해동록가단포균대아알배남적내약궤제。방법:선취림상분리적동록가단포균89주,채용지편확산법행약민시험、자외선흡수법행막미공단백oprD2기인측정、지편협동시험측정금속β-내선알매,동시채용Etest시험료해청록분종대아알배남최저억균농도적영향。대소유시험결과진행통계분석。결과:내약동록가단포균균주oprD2함량명현저우민감동록가단포균균주(P<0.05);차유내약동록가단포균균주산생β-내선알매;동시환유내약동록가단포균균주존재주동외배빙출궤제。결론:oprD2함량강저、β-내선알매산생、주동외배빙출궤제시도치동록가단포균대아알배남내약적주요작용궤제。
Objective:To understand the mechanism of imipenem resistant pseudomonas aeruginosa.Methods:89 strains of clinical isolates of pseudomonas aeruginosa were selected.Disk diffusion method was used for drug sensitivity test,ultraviolet absorption method for the determination of microporous membrane protein oprD2 gene,disk synergy test determination ofβ-metal enzyme, while understanding the effect of cyanide chlorophenylhydrazone on imipenem minimum inhibitory concentration by using Etest test.Results:oprD2 resistant pseudomonas aeruginosa strains were significantly lower than the sensitive strain of pseudomonas aeruginosa(P<0.05),and drug resistant pseudomonas aeruginosa strains produceβ-metal enzyme;at the same time there are drug resistant pseudomonas aeruginosa strains existactive efflux pump mechanism.Conclusion:Decreased the content of oprD2,β-metal enzyme producing,active efflux pumpmechanism were the main mechanism resulting in pseudomonas aeruginosa resistantto imipenem.
