目的:探讨实时剪切波弹性成像技术(SWE)在TI‐RADS 4级甲状腺单发实性结节中的应用价值。方法对251例甲状腺单发实性病灶患者行SWE检查,以病理结果为金标准对其及其中112例 TI‐RADS 4级结节分别绘制 ROC曲线,获取最佳界值E1、E2及曲线下面积ROC1、ROC2。结果 E1、E2分别为48.55 kPa和52.02 kPa。以E1为界值诊断T I‐RADS 4级甲状腺结节,敏感性、特异性分别为75.0%、77.7%,以E2为界值诊断 T I‐RADS 4级结节,敏感性、特异性分别为75.0%、78.3%,二者差异无统计学意义(P>0.05)。结论对于整体甲状腺结节确定的界值亦适用于TI‐RADS 4级结节,且SWE可在常规超声基础上进一步排除良、恶性病变,进一步指导临床。

목적:탐토실시전절파탄성성상기술(SWE)재TI‐RADS 4급갑상선단발실성결절중적응용개치。방법대251례갑상선단발실성병조환자행SWE검사,이병리결과위금표준대기급기중112례 TI‐RADS 4급결절분별회제 ROC곡선,획취최가계치E1、E2급곡선하면적ROC1、ROC2。결과 E1、E2분별위48.55 kPa화52.02 kPa。이E1위계치진단T I‐RADS 4급갑상선결절,민감성、특이성분별위75.0%、77.7%,이E2위계치진단 T I‐RADS 4급결절,민감성、특이성분별위75.0%、78.3%,이자차이무통계학의의(P>0.05)。결론대우정체갑상선결절학정적계치역괄용우TI‐RADS 4급결절,차SWE가재상규초성기출상진일보배제량、악성병변,진일보지도림상。
Objective To evaluate the diagnostic performance of shear wave elastography (SWE) for thyroid single solid nod‐ules .Methods 251 patients were performed of SWE examination .With the pathological results as gold standard ,ROC curves were drawn to obtain the thresholds (E1 ,E2 ) and diagnoses values (ROC1 ,ROC2 ) both for 251 patients and 112 patients of TI‐RADS 4 within them ,respectively .And the sensitivity and specificity were counted .Results The threshold value were 48 .55 kPa and 52 .02 kPa ,respectively .Using the E1 to diagnose the nodules of TI‐RADS 4 ,the sensitivity and specificity were 75 .0% and 77 .7% ,re‐spectively .Using the E2 to diagnose the nodules of TI‐RADS 4 ,the sensitivity and specificity were 75 .0% and 78 .3% ,respectively . And there was no significantly difference between the two sensitivities and specificities (P>0 .05) .Conclusion The threshold of all thyroid nodules was also apply to the nodules of TI‐RADS 4 .And SWE can significantly increase the diagnose value of thyroid nod‐ules by combining with conventional ultrasound .
