目的：探讨泼尼松加用益气补肾活血中药治疗慢性免疫性血小板减少症（CITP）的临床疗效和安全性。方法选择2011年1月－2013年12月新疆医科大学附属中医医院血液科收治的慢性免疫性血小板减少症患者120例，按自愿原则随机分为对照组（泼尼松）和观察组（泼尼松＋益气补肾活血中药），每组60例。对照组采用泼尼松1 mg·kg－1·d－1治疗；治疗组采用泼尼松治疗基础上，加益气补肾活血中药治疗。疗程结束后比较两组患者疗效、CD19＋／CD20＋淋巴细胞数及不良反应。结果对照组和观察组患者的总有效率分别为75．0％和95．0％，差异有统计学意义（P ＜0．05）。两组患者治疗后的 CD19＋／CD20＋淋巴细胞数均下降，差异有统计学意义（P ＜0．05），观察组患者较对照组患者下降更为明显，差异有统计学意义（P ＜0．05）。观察组患者血糖升高、血压升高、库欣样症状及神经精神症状等不良反应较对照组患者明显减少，差异有统计学意义（P ＜0．05）。结论采用泼尼松加用益气补肾活血中药是临床上治疗慢性免疫性血小板减少症患者更为安全有效的方案之一，值得临床积极推广应用。
목적：탐토발니송가용익기보신활혈중약치료만성면역성혈소판감소증（CITP）적림상료효화안전성。방법선택2011년1월－2013년12월신강의과대학부속중의의원혈액과수치적만성면역성혈소판감소증환자120례，안자원원칙수궤분위대조조（발니송）화관찰조（발니송＋익기보신활혈중약），매조60례。대조조채용발니송1 mg·kg－1·d－1치료；치료조채용발니송치료기출상，가익기보신활혈중약치료。료정결속후비교량조환자료효、CD19＋／CD20＋림파세포수급불량반응。결과대조조화관찰조환자적총유효솔분별위75．0％화95．0％，차이유통계학의의（P ＜0．05）。량조환자치료후적 CD19＋／CD20＋림파세포수균하강，차이유통계학의의（P ＜0．05），관찰조환자교대조조환자하강경위명현，차이유통계학의의（P ＜0．05）。관찰조환자혈당승고、혈압승고、고흔양증상급신경정신증상등불량반응교대조조환자명현감소，차이유통계학의의（P ＜0．05）。결론채용발니송가용익기보신활혈중약시림상상치료만성면역성혈소판감소증환자경위안전유효적방안지일，치득림상적겁추엄응용。
Objective To explore the clinical efficacy and safety of the treatment for chronic immune throm-bocytopenia by means of prednisone plus traditional Chinese medicine.Methods 120 cases with chronic immune thrombocytopenia in 2011.1-12 were selected from Hematology Department in Chinese Medicine Affiliated Hospital to Xinjiang Medical University.Control Group (prednisone)and Observation Group (prednisone plus Chinese medicine for replenishing qi and invigorating kidney),with 60 cases in each group,were randomly divided voluntarily.Control Group were treated with prednisone 1 mg·kg-1 ·d-1 treatment;Observation Group,with traditional Chinese medicine for the treatment of Qi added.CD19+/CD20+lymphocytes and adverse reactions in the two groups after treatment were compared and analyzed. Results The total effective rate in Control Group and Observation Group was 75.0% and 95.0% respec-tively,with statistical significance (P < 0.05 ).After treatment, the number of CD19 +/CD20 +lymphocyte in two groups decreased and the difference was statistically significant (P < 0.05 ),with greater decrease in the Observation Group than that in Control Group and the difference was statistically significant (P <0.05).The symptom of blood sugar,high blood pressure,Cushing-like symptoms,neu-ropsychiatric symptoms and other adverse reactions were significantly reduced in Observation Group when compared with those in Control Group,and the difference was statistically significant (P <0.05).Conclu-sions Prednisone plus Chinese medicine of replenishing qi to invigorate kidney is of safer and more effective clinical efficacy for chronic immune thrombocytopenia patients,which was worthy of clinical application.
