居家养老在养老服务中处于基础地位，无医疗功能的居家养老难以满足老年人养老需求，将医疗服务融入居家养老、发展医养结合居家养老模式有利于实现健康老龄化、积极老龄化。从居家养老的地位、养老机构提供医疗服务不足、居家养老中提供医疗服务优势等方面探讨居家医养结合的必要性；从政策环境的可行性、传统文化观念的可接受性、基层卫生资源的可获得性以及根据现有基本情况借鉴国内外经验四个方面分析居家医养结合的可行性；并从筹资来源、稳定人才、监督评估、试点推行等方面进一步探索医养结合居家养老模式的路径。

Home‐based care service is the fundamental service for the elderly . Home‐based care service without medical service cannot meet the demands of the elderly . Integration of medical service into home‐based care services and combination of medical care and nursing care are helping to the realization of healthy and positive ageing .This article discussed the necessity of combination of medical service and nursing care from three aspects ,such as the status of home‐based care service ,the inadequate medical care that pension institutions provided and the advantages of providing medical service in home‐based care service .It also analyzed the feasibility of combination of medical service and nursing care at home from four aspects ,such as the feasibility of the policy environment ,the acceptability of the traditional culture ideas , the availability of basic health resources and the experience according to at home and abroad for reference .And this essay tried to solve the existing problems from the sources of financing ,stable talent ,supervision and evaluation ,to further explore the path of combination model .
