医学人文具有理论和实践两个向度，其基础都在于临床实践。在医患矛盾较为突出、医患纠纷频发的背景下，促进临床医学人性化成为迫切需求，而沟通文化是促进医疗人性化的重要介质。南通大学附属医院以大医精神作为医患沟通的文化支点进行了大量实践。从沟通内容“直面焦点”、沟通常态化有赖“制度保障”、以“体验模式”提高沟通技能、沟通新载体借助“文化搭台”和“舆论先行”下的社会大沟通五个方面进行了论述，实践证明大医精神指导下的医患沟通效果提升了医院美誉度。

Medical humanities have two dimensions ,which are theory and practice ,and both of them are based on clinical practice . On the background of frequent patient‐doctor disputes and intense patient‐doctor conflicts ,it is urgent to promote clinical medicine to be human‐based , and communication culture is the important medium to fulfill this task . Affiliated Hospital of Nantong University has carried out lots of practice taking the great medical spirit as the cultural pivot for doctor‐patient communication . This article discussed the problem from five aspects such as facing up to the focus issues in communication content , the normalization of communication guaranteed by system ,promoting communication skill by experience mode ,having the aid of cultural stage to build new communication carrier ,the big social communication led by public opinion .T he practice proved that the doctor‐patient communication guided by the great medical spirit enhanced the hospital ’s reputation .
