目的:探讨下呼吸道烟曲霉菌烟曲霉菌耐药性和基因同源性。方法从31例下呼吸道烟曲霉菌感染患者的痰液或肺泡灌洗液中分离烟曲霉菌,采用CLSI M38-A2微量液基稀释法测定其对伏立康唑、伊曲康唑、两性霉素B、卡泊芬净、米卡芬净的敏感性;采用随机扩增DNA多态性( RAPD)基因分型技术获得受试菌株的RAPD指纹图并进行同源性聚类分析。结果31株受试烟曲霉对伏立康唑、伊曲康唑、两性霉素B、卡泊芬净、米卡芬净的耐药率分别为3.2%、12.9%、25.8%、0、0;31株烟曲霉共分为10种基因型(A~J),其中B型10株,为主要流行型别;其余分别为A型5株,E型3株,F型4株,C、H、I型各2株,D、G、J型各1株。结论下呼吸道烟曲霉菌感染患者烟曲霉菌基因型以B型为主,伏立康唑、卡泊芬净、米卡芬净对烟曲霉菌的抑菌效果较好。

목적:탐토하호흡도연곡매균연곡매균내약성화기인동원성。방법종31례하호흡도연곡매균감염환자적담액혹폐포관세액중분리연곡매균,채용CLSI M38-A2미량액기희석법측정기대복립강서、이곡강서、량성매소B、잡박분정、미잡분정적민감성;채용수궤확증DNA다태성( RAPD)기인분형기술획득수시균주적RAPD지문도병진행동원성취류분석。결과31주수시연곡매대복립강서、이곡강서、량성매소B、잡박분정、미잡분정적내약솔분별위3.2%、12.9%、25.8%、0、0;31주연곡매공분위10충기인형(A~J),기중B형10주,위주요류행형별;기여분별위A형5주,E형3주,F형4주,C、H、I형각2주,D、G、J형각1주。결론하호흡도연곡매균감염환자연곡매균기인형이B형위주,복립강서、잡박분정、미잡분정대연곡매균적억균효과교호。
Objective To study the resistance and homology of aspergillus fumigatus that caused lower respiratory tract infection.Methods The 31 clinical strains of aspergillus fumigatus were respectively isolated from the sputum and bronchoalveolar lavage fluid of patients who suffered from lower respiratory tract aspergillus fumigatus infection .The sus-ceptibility of five antifungal agents against aspergillus fumigatus was measured by liquid microdilution method and the ho -mology of strains was analyzed with random amplified polymorphic DNA (RAPD)assay.Results The resistance of the 31 tested aspergillus fumigatus strains was 3.2%to voriconazole,12.9%to itraconazole,25.8%to amphotericin B whereas all sensitive to caspofungin and micafungin.By RAPD,the 31 strains were divided into 10 genotypes(A~J),among which Type B was the major prevalent genotype that consisted of 10 strains.Conclusions The genotype B was proved to be the relevant type that caused lower respiratory tract aspergillus fumigatus infection in the surveyed hospital .Voriconazole ,caspo-fungin,micafungin exhibited favorable antifungal activity against Aspergillus fumigatus isolates in vitro .
