目的:分析血糖波动对重症监护病房危重患者预后的影响,探讨血糖波动与患者预后的相关性。方法回顾性分析82例重症监护病房内的危重症患者的临床资料,根据存活情况将患者分为存活组(n=32)和死亡组(n=50),对比2组患者入院时即时血糖(GluAd)、住院期间平均血糖(GluAv)、住院期间血糖波动幅度(GluSD)及血糖变异系数(GluCV)差异。同时分析GluAd、GluAv、GluSD及GluCV对患者预后的预测作用。结果2组GluAd及GluAv比较差异无统计学意义,但死亡组GluSD及GluCV则显著高于生存组(P<0.05)。同时经ROC分析显示,GluAd及GluAv对于患者预后无预测作用,而GluSD及GluCV对患者的预后存在预测作用(P<0.05),且GluCV的预测作用更准确(P<0.05)。结论血糖波动对重症监护病房为危重症患者的预后有着明显的预测作用,且以GluCV的预测作用更为准确。

목적:분석혈당파동대중증감호병방위중환자예후적영향,탐토혈당파동여환자예후적상관성。방법회고성분석82례중증감호병방내적위중증환자적림상자료,근거존활정황장환자분위존활조(n=32)화사망조(n=50),대비2조환자입원시즉시혈당(GluAd)、주원기간평균혈당(GluAv)、주원기간혈당파동폭도(GluSD)급혈당변이계수(GluCV)차이。동시분석GluAd、GluAv、GluSD급GluCV대환자예후적예측작용。결과2조GluAd급GluAv비교차이무통계학의의,단사망조GluSD급GluCV칙현저고우생존조(P<0.05)。동시경ROC분석현시,GluAd급GluAv대우환자예후무예측작용,이GluSD급GluCV대환자적예후존재예측작용(P<0.05),차GluCV적예측작용경준학(P<0.05)。결론혈당파동대중증감호병방위위중증환자적예후유착명현적예측작용,차이GluCV적예측작용경위준학。
Objective To analyze the impact of fluctuations in blood glucose in critically ill patients on the ICU prognosis investigate lfuctuations in blood sugar and prognostic relevance.Methods A retrospective analysis of the clinical data of 82 cases of critically ill patients in the intensive care unit, according to the survival of the patients were divided into survival group (32 patients) and death group (50 patients), compared two groups of patients admitted to hospital immediately when blood sugar (GluAd), average blood glucose during hospitalization (GluAv), during hospitalization blood sugar fluctuations (GluSD) and glucose variability coefficient (GluCV) differences. Simultaneous analysis predict GluAd, GluAv, GluSD and GluCV on prognosis.Results There was no statistically signiifcant difference groups GluAd and GluAv, but the death group GluSD and GluCV signiifcantly higher than the survival group (P<0.05). Meanwhile by ROC analysis showed that, GluAd and GluAv no predictor of prognosis for patients, while GluSD and GluCV to predict the prognosis of patients exists (P<0.05), and a more accurate predictor of GluCV (P<0.05). Conclusion Blood sugar lfuctuations on the ICU for prognosis of critically ill patients have signiifcant predictive effect, and to more accurately predict GluCV.
