目的：探讨联合血液净化重症急性胰腺炎的临床疗效。方法40例SAP患者按入院早晚随机分2组，常规治疗组（n=18）和CBP（continuous blood purification）组（n=22）。观察各组治疗前后以及2组间APACHEI 评分、血清淀粉酶、SCr、CRP、WBC、BE、SaO 2的变化情况。结果治疗后，2组的AMS、SCr、CRP、WBC、BE、SaO 2值均低于治疗前（P<0.05）；治疗后，CBP组AMS、SCr、CRP、WBC、BE、SaO 2值显著低于常规治疗组（P<0.05）；治疗后，2组的APACHEI评分均显著低于治疗前（P<0.05）；治疗后，CBP组APACHEI 得分与常规治疗组相比，差异无统计学意义。结论在常规治疗基础上联合血液净化治疗重症急性胰腺炎效果明显，值得临床进一步推广。
목적：탐토연합혈액정화중증급성이선염적림상료효。방법40례SAP환자안입원조만수궤분2조，상규치료조（n=18）화CBP（continuous blood purification）조（n=22）。관찰각조치료전후이급2조간APACHEI 평분、혈청정분매、SCr、CRP、WBC、BE、SaO 2적변화정황。결과치료후，2조적AMS、SCr、CRP、WBC、BE、SaO 2치균저우치료전（P<0.05）；치료후，CBP조AMS、SCr、CRP、WBC、BE、SaO 2치현저저우상규치료조（P<0.05）；치료후，2조적APACHEI평분균현저저우치료전（P<0.05）；치료후，CBP조APACHEI 득분여상규치료조상비，차이무통계학의의。결론재상규치료기출상연합혈액정화치료중증급성이선염효과명현，치득림상진일보추엄。
Objective To evaluate the clinical curative effects of joint blood puriifcation therapy in treatment severe acute pancreatitis.Methods 40 cases of SAP patients in our hospital were selected and randomly divided into conventional treatment group (n=18) and continuous blood puriifcation group (CBP) (n=22), by hospitalization time. The APACHEII score, AMS, SCr, CRP, WBC, BE, SaO2 changes of pre-treatment and post-treatment of the two groups were observed.Results After treatment, AMS, SCr, CRP, WBC, BE, SaO2 values of post-treatment in two groups were all lower than those of pre-treatment (P<0.05). After treatment, AMS, SCr, CRP, WBC, BE, SaO2 values of CBP group were all lower than those of conventional treatment group (P<0.05).After treatment, APACHEII scores of post-treatment in two groups were all lower than those of pre-treatment (P<0.05). After treatment, APACHEII scores of two groups had non signiifcant difference.Conclusion The joint blood puriifcation therapy based on the conventional treatment has a signiifcant effects on severe acute pancreatitis patients,and is worthy of clinical promotion.
