The third generation of EMS%Nephroscope%Intrahepatic bile duct stone%Porphyrization and clearance of calculus
目的：探求治疗肝内胆管结石的一种手术方式。方法在彩超引导下穿刺含结石肝内胆管，置入斑马导丝，扩张穿刺道，置入肾镜，第三代EMS的超声探针经肾镜碎石并同时取出碎石，术毕置引流管于肝内胆管。结果21例肝内胆管结石手术顺利完成，无严重并发症及结石残留。结论第三代EMS经肾镜肝内胆管结石碎石清石手术安全、有效。
目的：探求治療肝內膽管結石的一種手術方式。方法在綵超引導下穿刺含結石肝內膽管，置入斑馬導絲，擴張穿刺道，置入腎鏡，第三代EMS的超聲探針經腎鏡碎石併同時取齣碎石，術畢置引流管于肝內膽管。結果21例肝內膽管結石手術順利完成，無嚴重併髮癥及結石殘留。結論第三代EMS經腎鏡肝內膽管結石碎石清石手術安全、有效。
목적：탐구치료간내담관결석적일충수술방식。방법재채초인도하천자함결석간내담관，치입반마도사，확장천자도，치입신경，제삼대EMS적초성탐침경신경쇄석병동시취출쇄석，술필치인류관우간내담관。결과21례간내담관결석수술순리완성，무엄중병발증급결석잔류。결론제삼대EMS경신경간내담관결석쇄석청석수술안전、유효。
Objective To investigate a kind of operation method in the management of intrahepatic bile duct stone. Methods To utilize paracentesis into the intrahepatic bile duct with calculus by color Doppler echocardiograph guided, insert zebra guide wire, dilate pricking canal and insert nephroscope. The intrahepatic bile duct stone was to be crushed to pieces and precluded cleanly by ultrasound exploring needle with third generation of EMS inserted. The drainage tube was inserted into intrahepatic bile duct after paracentesis procedure. Results The operation was completed smoothly in the 21 patients with intrahepatic bile duct stone. There was not any severe complication and residual calculus. Conclusion The porphyrization and clearance of intrahepatic bile duct stone by the third generation of EMS with nephroscope is safely and effectively.
