目的：探讨2型糖尿病（T2DM）患者血清CA199与血糖控制水平的相关性及临床意义。方法收集52例健康体检者（A组，对照组）及146例住院2型糖尿病患者，其中糖尿病患者按HbAlc水平分为控制组（B组，HbA1c＜7.5％）及未控制组（C组，HbA1c＞7.5％）两组。比较3组CA199水平；分析HbA1c与CA199的相关性；以性别、年龄、病程、体重指数、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、空腹血糖（FPG）、餐后2 h血糖（2hPG）、HbA1c为自变量，进行多元逐步回归分析，比较各变量对CA199的影响程度。结果 C组的CA199水平显著高于B组及A组（P＜0.05），而B组及A组CA199水平差异无统计学意义（P＞0.05）；相关分析显示CA199与HbA1c、FPG、2hPG、TG均呈正相关。结论血糖未控制的T2DM患者CA199水平明显高于血糖控制患者及健康者；HbA1c是影响CA199的独立危险因素，T2DM患者CA199升高与血糖长期控制差有密切关系。
목적：탐토2형당뇨병（T2DM）환자혈청CA199여혈당공제수평적상관성급림상의의。방법수집52례건강체검자（A조，대조조）급146례주원2형당뇨병환자，기중당뇨병환자안HbAlc수평분위공제조（B조，HbA1c＜7.5％）급미공제조（C조，HbA1c＞7.5％）량조。비교3조CA199수평；분석HbA1c여CA199적상관성；이성별、년령、병정、체중지수、총담고순、감유삼지、고밀도지단백담고순、저밀도지단백담고순、공복혈당（FPG）、찬후2 h혈당（2hPG）、HbA1c위자변량，진행다원축보회귀분석，비교각변량대CA199적영향정도。결과 C조적CA199수평현저고우B조급A조（P＜0.05），이B조급A조CA199수평차이무통계학의의（P＞0.05）；상관분석현시CA199여HbA1c、FPG、2hPG、TG균정정상관。결론혈당미공제적T2DM환자CA199수평명현고우혈당공제환자급건강자；HbA1c시영향CA199적독립위험인소，T2DM환자CA199승고여혈당장기공제차유밀절관계。
Objective To investigate the correlation between serum CA-199 and blood glucose control in patients with type 2 diabe-tes and its clinical significance.Methods 52 healthy subjects (group A,control group)and 146 inpatients with type 2 diabetes were en-rolled in the study.All the diabetic inpatients were divided into controlled group (group B,HbA1c <7.5%)and uncontrolled group (group C,HbA1c >7.5%)according to the HbA1c level.The levels of serum CA-199 among three groups were compared,and the relationship be-tween levels of HbA1 c and CA-l99 was analyzed.Multiple stepwise regression analysis was performed to compare the effect of different inde-pendent variables on CA-199,and the independent variables were sex,age,duration,TC,TG,HDL,LDL,FPG,2hPG and HbA1c.Re-sults In group C,the level of CA-l99 was significantly higher than that in group B and group A(P<0.05 ),while there was no significant difference between group B and group A(P>0.05).Correlation analysis showed that the level of CA-199 was positively related to the levels of HbA1c,FPG,2hPG and TG.Conclusion The CA-199 levels in patients with type 2 diabetes in poor glucose control are significantly higher than those in well controlled patients and healthy subjects.HbA1c might be an independent risk factor of CA-199,since the increase of CA-199 level is closely related to long-term poor glucose control in patients with type 2 diabetes.
