目的:探讨小剂量氨茶碱对急性颈髓损伤后心动过缓患者的疗效。方法42例急性颈髓损伤后心动过缓患者,首次静脉注射氨茶碱负荷量5 mg/kg(10~30 min),之后0.5 mg/(kg·h)持续静脉泵注10~14 d。治疗期间每2天监测茶碱血药浓度。分析治疗有效率,同时观察和记录患者的呕吐、躁动和抽搐不良反应发生情况。结果42例患者中37例有效,5例无效,有效率为88.10%。治疗后HR(70.00±5.42)次/min,明显高于治疗前(45.00±6.28)次/min(t=23.93, P<0.01)。茶碱血药浓度为1.55~5.27 mg/L。不良反应发生情况:呕吐2例、躁动2例,无抽搐,总体不良反应发生率为9.52%。结论小剂量氨茶碱能够有效治疗颈髓损伤后心动过缓。

목적:탐토소제량안다감대급성경수손상후심동과완환자적료효。방법42례급성경수손상후심동과완환자,수차정맥주사안다감부하량5 mg/kg(10~30 min),지후0.5 mg/(kg·h)지속정맥빙주10~14 d。치료기간매2천감측다감혈약농도。분석치료유효솔,동시관찰화기록환자적구토、조동화추휵불량반응발생정황。결과42례환자중37례유효,5례무효,유효솔위88.10%。치료후HR(70.00±5.42)차/min,명현고우치료전(45.00±6.28)차/min(t=23.93, P<0.01)。다감혈약농도위1.55~5.27 mg/L。불량반응발생정황:구토2례、조동2례,무추휵,총체불량반응발생솔위9.52%。결론소제량안다감능구유효치료경수손상후심동과완。
Objective To explore the curative effect of small dose of aminophylline in the treatment of bradycardia after acute cervical spinal cord injury. Methods There were 42 patients with bradycardia after acute cervical spinal cord injury. The load capacity of the first intravenous injection of aminophylline was 5 mg/kg (10~30 min). After that, the dose was 0.5 mg/(kg·h) for 10~14 d by continuous intravenous pump injection. During the treatment, theophylline blood concentration was detected in every 2 days, and the effective rate was analyzed. Observation and records were made on adverse reactions of emesis, dysphoria, and convulsion. Results Among the 42 cases, there were 37 effective cases and 5 ineffective cases, and the effective rate was 88.10%. After the treatment, HR was (70.00±5.42) times/min, which was obviously higher than (45.00±6.28) times/min before treatment (t=23.93, P<0.01). The theophylline blood concentration was 1.55~5.27 mg/L. There were 2 cases with emesis, 2 cases with dysphoria, and none with convulsion as adverse reactions. The total incidence of adverse reactions was 9.52%. Conclusion Small dose of aminophylline can effectively treat bradycardia after acute cervical spinal cord injury.
