目的:探讨女性不同生理期乳腺彩色多普勒超声声像图的临床应用效果。方法选取400例女职工体检,按照年龄分为青春发育期、育龄期、妊娠晚期、哺乳期、绝经后老年期5组,分别为79例、158例、49例、35例、79例。探头频率9~14 MHz,每受检人员均以乳头为中心,左侧乳房在位于1点处,右侧乳房在位于11点处测量皮下脂肪与腺体厚度;测乳头、乳晕交换处的导管内径,观察分析不同生理期声像图表现,记录存档。结果通过本研究发现,青春发育期的乳腺为导管Ⅰ型较多,约占37.9%;育龄期不均型较多,占44.9%;妊娠晚期和哺乳期以导管Ⅱ型为主,比例分别为44.8%、71.4%;绝经老年期的乳腺则是不均型和紊乱型为主,比例分别为34.2%和35.6%;并统计出乳腺各生理期腺体厚度和导管内径的变化以及正常值范围。结论本项研究认为在乳腺普查中,不论仰卧位或侧卧位均能较为准确的测量皮下脂肪及腺体厚度、导管内径,为今后的乳腺普查,奠定了基础,快捷方便,缩短时间,减少程序。

목적:탐토녀성불동생리기유선채색다보륵초성성상도적림상응용효과。방법선취400례녀직공체검,안조년령분위청춘발육기、육령기、임신만기、포유기、절경후노년기5조,분별위79례、158례、49례、35례、79례。탐두빈솔9~14 MHz,매수검인원균이유두위중심,좌측유방재위우1점처,우측유방재위우11점처측량피하지방여선체후도;측유두、유훈교환처적도관내경,관찰분석불동생리기성상도표현,기록존당。결과통과본연구발현,청춘발육기적유선위도관Ⅰ형교다,약점37.9%;육령기불균형교다,점44.9%;임신만기화포유기이도관Ⅱ형위주,비례분별위44.8%、71.4%;절경노년기적유선칙시불균형화문란형위주,비례분별위34.2%화35.6%;병통계출유선각생리기선체후도화도관내경적변화이급정상치범위。결론본항연구인위재유선보사중,불론앙와위혹측와위균능교위준학적측량피하지방급선체후도、도관내경,위금후적유선보사,전정료기출,쾌첩방편,축단시간,감소정서。
Objective To research clinical application of color Doppler images of galactophore according to different menstrual periods of women. Methods Selected 400 cases of female medical workers, according to age into adolescence, childbearing, late pregnancy, lactation, old age postmenopausal ifve groups were 79 cases, 158 cases, 49 cases, 35 cases, 79 cases. Probe frequency 9~14 MHz, subjects who are each nipple as the center, located in the left breast at 1:00, right breast at 11:00 located at measuring the thickness of subcutaneous fat and glands;measuring nipple, areola exchange at the inner diameter of the catheter observe different physiological analysis of sonography, documented.Results According to this research, the galactophore in the period of adolescence mainly belongs to Catheter I with the proportion of 32.9%. For people in the child-bearing period, 44.9%of them are heterogeneous. In the period of lactation and menopause, most of the galactophore belong to Catheter II with the proportion of 44.8%and 71.0%. In the menopause period, 34.2%of them are heterogeneous, and 35.6%of them are inordinate.The variations of the glandular thickness and the internal catheter diameter were worked out as well as the range of normal value.Conclusion In breast screening, the new method proposed has a better efifciency.
