目的：探讨锌辅助治疗在小儿重症肺炎中的临床意义及可能机制。方法80例小儿重症肺炎随机分为实验组和对照组各40例，两组均给予常规治疗，在此基础上实验组给予葡萄糖酸锌片70 mg/片口服，早晚各一次，对照组给予给予安慰剂口服，早晚各一次，观察两组治疗效果及免疫功能变化情况。结果实验组总有效率为95.00%；对照组总有效率为75.00%，两组比较差异有明显的统计学意义(χ2=6．27；P<0．05)，实验组患儿发热消失时间(2．43±0．64d)，咳嗽消失时间(4．44±1．86d)，肺部啰音消失时间(6．34±2．12d)及住院时间(11．23±4．33d)均明显短于对照组患儿(3．44±0．87d；6．34±2．12 d；8．34±2．39 d；14．34±4．37 d)，差异均有明显的统计学意义(P<0．05)；并且治疗后试验组组患儿IgA水平(1．94±0．19 g/L )明显高于对照组(1．65±0．16 g/L )和治疗前(1．53±0．14g/L )，差异均有明显的统计学意义(P<0．05)。结论锌辅助治疗小儿重症肺炎有助于缩短疗程，其机理可能与锌制剂调节免疫有关。
목적：탐토자보조치료재소인중증폐염중적림상의의급가능궤제。방법80례소인중증폐염수궤분위실험조화대조조각40례，량조균급여상규치료，재차기출상실험조급여포도당산자편70 mg/편구복，조만각일차，대조조급여급여안위제구복，조만각일차，관찰량조치료효과급면역공능변화정황。결과실험조총유효솔위95.00%；대조조총유효솔위75.00%，량조비교차이유명현적통계학의의(χ2=6．27；P<0．05)，실험조환인발열소실시간(2．43±0．64d)，해수소실시간(4．44±1．86d)，폐부라음소실시간(6．34±2．12d)급주원시간(11．23±4．33d)균명현단우대조조환인(3．44±0．87d；6．34±2．12 d；8．34±2．39 d；14．34±4．37 d)，차이균유명현적통계학의의(P<0．05)；병차치료후시험조조환인IgA수평(1．94±0．19 g/L )명현고우대조조(1．65±0．16 g/L )화치료전(1．53±0．14g/L )，차이균유명현적통계학의의(P<0．05)。결론자보조치료소인중증폐염유조우축단료정，기궤리가능여자제제조절면역유관。
Objective To evaluate the clinical significance of zinc-assisted treatment for children with seri-ous pneumonia. Methods 80 children with serious pneumonia were randomly divided into the experimental group and the control group. All children were treated with primary treatment. The experimental group was treated with zinc gluconate tablets (70mg), and the control group was treated with a placebo. The treatment effectiveness and immune function were observed. Results The total effective rate was 95. 00% in the experimental group and 75. 00% in the control group (χ2 = 6. 27, P<0. 05). The fever disappearance time was (2. 43 ± 0. 64d), the cough disappearance time was (4. 44 ± 1. 86d), the pulmonary rale disappeared time was (6. 34 ± 2. 12d) and the duration of hospital stay was (11. 23 ± 4. 33d) in the experimental group, which were significantly shorter than in the control group (P<0. 05). Conclusion The adjuvant therapy with zinc gluconate can shorten the treatment course in the treatment of children with serious pneumonia.
