目的：探讨微创经皮穿刺牛角扩张气管造口术在危重患者中的临床应用意义。方法40例患者根据选取气管切开方式随机分为微创牛角扩张气管造口术组20例及开放性气管切开术组20例。比较两组手术操作时间、出血量、切口长度、切口愈合时间以及相关并发症和安全性等差异。结果微创牛角扩张气管造口术组在手术时间、出血量、切口大小、切口愈合时间及术后并发症发生率等均显著优于开放性气管切开术组，术前两组生命体征安全性观察指标比较，无明显差异(P>0．05)，术后观察指标组间比较，研究组比对照组术后生命体征波动变化小，差异具有统计学意义(P<0．05)。结论微创牛角扩张气管造口术具有安全快速、创伤小、并发症发生率低、手术操作简单等优点，值得在危重患者中广泛应用。
목적：탐토미창경피천자우각확장기관조구술재위중환자중적림상응용의의。방법40례환자근거선취기관절개방식수궤분위미창우각확장기관조구술조20례급개방성기관절개술조20례。비교량조수술조작시간、출혈량、절구장도、절구유합시간이급상관병발증화안전성등차이。결과미창우각확장기관조구술조재수술시간、출혈량、절구대소、절구유합시간급술후병발증발생솔등균현저우우개방성기관절개술조，술전량조생명체정안전성관찰지표비교，무명현차이(P>0．05)，술후관찰지표조간비교，연구조비대조조술후생명체정파동변화소，차이구유통계학의의(P<0．05)。결론미창우각확장기관조구술구유안전쾌속、창상소、병발증발생솔저、수술조작간단등우점，치득재위중환자중엄범응용。
Objective To explore the significance of expansion of minimally invasive percutaneous tracheos-tomy horn expansion in the treatment of critically ill patients. Methods 40 patients were randomly divided into the invasive horn expansion tracheostomy study group ( 20 cases ) and the open tracheotomy tracheotomy control group (20 cases). The operative time, blood loss, length of incision, wound healing time and complications, and safety were evaluated between the two groups. Results The improvement of operative time, blood loss, incision size, wound healing time and postoperative complication rate was more pronounced in the study group than in the control group (P<0. 01). There was no significant difference in vital signs between the two groups before the treatment (P>0. 05), but it showed significant difference after the treatment (P<0. 05). Conclusion Minimally invasive tra-cheostomy horns expansion has the advantages of high safety, convenience, low complication rate and easy operation in the treatment of critically ill patients.
