目的：对比七叶皂苷与丹参注射液治疗四肢创伤及术后肿胀的临床效果及安全性。方法将2011年3月~2013年3月本院收治的四肢创伤及术后肿胀的84例患者随机分为观察组和对照组，其中观察组42例采用七叶皂苷消肿治疗，对照组42例采用丹参注射液消肿治疗，比较两组的临床疗效，治疗7 d后的肿胀评分、疼痛评分、患肢周径，不良反应发生率。结果两组的总有效率比较差异有统计学意义(P<0.05)；两组治疗7 d后的肿胀评分、疼痛评分及患肢周径差异均有统计学意义(P<0.05)；用药过程中，两组均有刺激性疼痛，经对症处理后好转，两组的不良反应发生率差异有统计学意义(P<0.05)。结论七叶皂苷具有明显的消肿作用，治疗四肢创伤及术后肿胀的临床效果好，不良反应发生率低，值得临床推广。

목적：대비칠협조감여단삼주사액치료사지창상급술후종창적림상효과급안전성。방법장2011년3월~2013년3월본원수치적사지창상급술후종창적84례환자수궤분위관찰조화대조조，기중관찰조42례채용칠협조감소종치료，대조조42례채용단삼주사액소종치료，비교량조적림상료효，치료7 d후적종창평분、동통평분、환지주경，불량반응발생솔。결과량조적총유효솔비교차이유통계학의의(P<0.05)；량조치료7 d후적종창평분、동통평분급환지주경차이균유통계학의의(P<0.05)；용약과정중，량조균유자격성동통，경대증처리후호전，량조적불량반응발생솔차이유통계학의의(P<0.05)。결론칠협조감구유명현적소종작용，치료사지창상급술후종창적림상효과호，불량반응발생솔저，치득림상추엄。
Objective To compare the clinical effect and safety of seven leaf saponin and Danshen injection in the treatment of limbs’ trauma and swelling after operation. Methods 84 cases with limbs’ trauma and swelling after opera-tion treated in our hospital from March 2011 to March 2013 were randomly divided into the observation group and the control group,and patients in the observation group (n=42) were treated with seven leaf saponin for detumescence,while patients in the control group (n=42) were treated with Danshen injection for detumescence.The clinical curative effect, swelling score,pain score,limb circumference after 7 days treatment and the incidence rate in the two groups were com-pared. Results There was a statistical difference of the total effective rate in the two groups (P<0.05);there was a statis-tical difference of swelling score,pain score,limb circumference after 7 days treatment in the two groups (P<0.05);in the process of treatment,there was stimulation of pain cured after symptomatic treatment in the two groups,and there was a statistical difference of the incidence rate of complication in the two groups (P<0.05). Conclusion Seven leaf saponin has the obvious effect of detumescence,and it has good clinical effect treating limbs’ trauma and swelling after opera-tion with lower the incidence rate of complication.It is worthy of clinical promotion.
