目的:探讨右美托咪定( Dex)在下腹部手术全麻患者苏醒期的应用效果。方法将2012年12月—2013年12月全麻行下腹部手术的患者76例依据临床麻醉方式分为A组19例、B组19例、C组20例和D组18例,A组不给予Dex,B、C、D组分别给予Dex 0.3、0.5、1.0μg/kg。观察4组心率、血压及血氧饱和度(SpO2)情况,比较镇痛效果以及不良反应发生情况。结果 C、D组心率、血压及SpO2均低于A组,Ramsay评分高于A组,SAS和VRS评分均低于A组(P<0.05);D组心动过缓和苏醒延迟发生率高于A组(P<0.05)。结论中、高剂量Dex均具有良好的麻醉效果,采用0.5 μg/kg安全性较高。

목적:탐토우미탁미정( Dex)재하복부수술전마환자소성기적응용효과。방법장2012년12월—2013년12월전마행하복부수술적환자76례의거림상마취방식분위A조19례、B조19례、C조20례화D조18례,A조불급여Dex,B、C、D조분별급여Dex 0.3、0.5、1.0μg/kg。관찰4조심솔、혈압급혈양포화도(SpO2)정황,비교진통효과이급불량반응발생정황。결과 C、D조심솔、혈압급SpO2균저우A조,Ramsay평분고우A조,SAS화VRS평분균저우A조(P<0.05);D조심동과완화소성연지발생솔고우A조(P<0.05)。결론중、고제량Dex균구유량호적마취효과,채용0.5 μg/kg안전성교고。
Objective To investigate the effect of Dexmedetomidine ( Dex) on the stage of analepsia of patients un-dergoing lower abdominal operation under general anaesthesia. Methods A total of 76 patients under general anaesthesia un-dergoing lower abdominal operation during December 2012 and December 2013 were divided into group A (n=19), group B (n=19), group C (n=20) and group D (n=18) according to anesthesia methods. Group A was not given Dex treatment, while group B, C and D were given Dex 0. 3, 0. 5 and 1. 0μg/kg treatment. The heart rate, blood pressure and blood oxygen saturation ( SpO2 ) in the four groups were observed, and analgesic effects and incidence rates of adverse reactions in the four groups were compared. Results In group C and D, the values of heart rate, blood pressure and SpO2 were significantly low-er, while the Ramsay scores were significantly higher, but the scores of sedation-agitation scale ( SAS) and verbal rating scales (VRS) were significantly lower than those in group A (P<0. 05);incidence rates of bradycardia and delayed recovery in group D were significantly higher than those in group A (P<0. 05). Conclusion Medium and large doses of Dex may a-chieve good anesthetic effect, and the Dex 0. 5 μg/kg is safe.
