目的：探讨银离子藻酸盐抗菌敷料对腿部静脉性溃疡的减痛促愈效果。方法选择2012年2月~2013年12月于宜昌市中心人民医院伤口治疗门诊就诊的腿部静脉溃疡患者75例,将入选患者随机分为A、B、C三组,分别使用进口的银离子藻酸盐敷料、国产纳米银敷料和常规碘伏敷料处理腿部溃疡,在设定的研究期内,治疗10、20、30 d摄取照片计算溃疡面积缩小率,评估更换敷料时和两次更换敷料间患者的疼痛评分、敷料的二次创伤积分。结果三组患者均完成研究,A组患者不同时间溃疡面积缩小率、更换敷料时和两次更换敷料间的疼痛评分、敷料二次创伤积分均优于B组和C组,差异有统计学意义(P<0.05)；B组面积缩小率和疼痛评分明显优于C组(P<0.01),但B、C两组敷料二次创伤积分基本一致,差异无统计学意义(P>0.05)。结论两种银敷料处理腿部静脉性溃疡的减痛促愈效果明显优于常规碘伏敷料,银离子藻酸盐抗菌敷料对溃疡伤口床的改善及减痛促愈作用比纳米银敷料更优越。
목적：탐토은리자조산염항균부료대퇴부정맥성궤양적감통촉유효과。방법선택2012년2월~2013년12월우의창시중심인민의원상구치료문진취진적퇴부정맥궤양환자75례,장입선환자수궤분위A、B、C삼조,분별사용진구적은리자조산염부료、국산납미은부료화상규전복부료처리퇴부궤양,재설정적연구기내,치료10、20、30 d섭취조편계산궤양면적축소솔,평고경환부료시화량차경환부료간환자적동통평분、부료적이차창상적분。결과삼조환자균완성연구,A조환자불동시간궤양면적축소솔、경환부료시화량차경환부료간적동통평분、부료이차창상적분균우우B조화C조,차이유통계학의의(P<0.05)；B조면적축소솔화동통평분명현우우C조(P<0.01),단B、C량조부료이차창상적분기본일치,차이무통계학의의(P>0.05)。결론량충은부료처리퇴부정맥성궤양적감통촉유효과명현우우상규전복부료,은리자조산염항균부료대궤양상구상적개선급감통촉유작용비납미은부료경우월。
Objective To observe the effects of silver ion alginate antimicrobial dressings on healing and reducing leg venous ulcer. Methods 75 cases of patients with leg venous ulcers treated in the Wound Therapy Outpatient Clinic of Central People's Hospital of Yichang City from February 2012 to December 2013 were randomly divided into group A, group B, and group C. Group A was treated with imported silver ions alginate dressings; group B was treated with do-mestic nanometer silver dressings, group C was treated with conventional iodine dressings. In the set research period, twice dressings' trauma scores and the patients' pain scores caursed by changing dressings and during two changes of dressings were evaluated;calculation of ulcer area reduction rate was made by taking photos on the treatment of 10, 20 d, and 30 d. Results The study of three groups has been completed, the ulcer area reduction rate, twice dressings' trauma scores and the patients' pain saores coursed by changing dressings and during two changes of dressings of the group A were better than those of the group B and group C, the differences were statistical significance (P < 0.05); the ulcer area reduction rate and patients' pain scores were better than those of the group C (P<0.01); but the twice dressings' trauma scores of the group B and group C were almost the same, there was no statistical significance (P> 0.05). Con-clusion The effects of two silver dressings on healing and reducing the pains of leg venous ulcer are much better than that of iodine dressings, and the effects of silver ions alginate dressings on healing and reducing the pains of the ulcer are much better than that of nanometer silver dressings.
