目的：探讨医院用药错误（ME）原因，提出针对性地改进措施。方法选取2012－2013年我院ME 事件75例进行汇总、归类和分析，包括ME分类、分级、报告人及有关构成比。结果 ME分类中处方差错、遗漏错误、剂量错误和用药时间错误居前4位，ME分级中主要以用药错误为差错隐患和轻型ME为主，药师和护士为主要报告人。结论应加强信息化建设，建立制度，增进医护药患协同，减少用药错误，保障用药安全。
목적：탐토의원용약착오（ME）원인，제출침대성지개진조시。방법선취2012－2013년아원ME 사건75례진행회총、귀류화분석，포괄ME분류、분급、보고인급유관구성비。결과 ME분류중처방차착、유루착오、제량착오화용약시간착오거전4위，ME분급중주요이용약착오위차착은환화경형ME위주，약사화호사위주요보고인。결론응가강신식화건설，건립제도，증진의호약환협동，감소용약착오，보장용약안전。
Objective To explore the causes of hospital medication errors,put forward specific improvement measures.Methods 75 cases were colected,classified and analyzed from 2012 to 2013 in our hospital ME events,including the ME classification,grading,reports of people and relevant constituent ratio.Results ME classification of prescription errors,omissions,wrong dose and medication time error is ranked in the top 4,ME grading is mainly in medication errors for the hidden mistake and light ME,pharmacists and nurses as the main report. Conclusion Strengthen the construction of information,the establishment of the system,improve the medical drug patient cooperation,reduce medication errors, ensure drug safety.
