目的:分析探讨急性心肌梗死(AMI)患者血清中心型脂肪酸结合蛋白(H-FABP)、肌钙蛋白I(cTnI)、肌酸激酶同功酶(CK-MB)动态变化情况。方法将2012年1月~2013年12月期间收治的142例AMI患者作为观察组,选择同期健康人员100例为对照组,对比分析两组的H-FABP、cTnI以及CK-MB的水平。结果观察组的H-FABP、cTnI以及CK-MB的平均浓度均高于对照组(P<0.05);观察组H-FABP在0~4 h上升,6~8 h达峰值,24~30 h后回至正常;cTnI在4~8 h上升,10~12 h达峰值,60 h后出现回落;CK-MB在4~8 h上升,10~15 h达高峰,3 d后回至正常。结论采用H-FABP、cTnI以及CK-MB检测AMI各具优势,H-FABP、cTnI以及CK-MB联合检测可提高AMI早期诊断的敏感性,提倡在临床上推广应用。

목적:분석탐토급성심기경사(AMI)환자혈청중심형지방산결합단백(H-FABP)、기개단백I(cTnI)、기산격매동공매(CK-MB)동태변화정황。방법장2012년1월~2013년12월기간수치적142례AMI환자작위관찰조,선택동기건강인원100례위대조조,대비분석량조적H-FABP、cTnI이급CK-MB적수평。결과관찰조적H-FABP、cTnI이급CK-MB적평균농도균고우대조조(P<0.05);관찰조H-FABP재0~4 h상승,6~8 h체봉치,24~30 h후회지정상;cTnI재4~8 h상승,10~12 h체봉치,60 h후출현회락;CK-MB재4~8 h상승,10~15 h체고봉,3 d후회지정상。결론채용H-FABP、cTnI이급CK-MB검측AMI각구우세,H-FABP、cTnI이급CK-MB연합검측가제고AMI조기진단적민감성,제창재림상상추엄응용。
Objective To observe and analyze dynamic changes of serum heart-type fatty acid-binding protein(H-FABP),cardiac troponin I(cTnI), creatine kinase mb(CK-MB)in the patients with acute myocardial infarction(AMI),then figure out the relationship between them and the development of the disease. Methods 142 cases of the AMI patients were selected as treatment group from January 2012 to December 2012,and 100 cases of health people were selected as the control group in the same period. Compared level of H-FABP,cTnI and CK-MB of two groups of subjects. Results The average concentration of H-FABP,cTnI and CK-MB of treatment group were higher than those of control group(P<0.05); The concentration of H-FABP of treatment group increased in 0~4 h,6~8 h to peake value,and was back to normal after 24~30 h;The concentration of cTnI increased in 4~8 h,10~12 h to peake value,and was back to normal after 60 h;and the CK-MB was in 4~8 h,10~15 h to peake value,back to normal after 3 d. Conclusion The concentration changes of H-FABP,cTnI and CK-MB to detect AMI had their respective advantages. H-FABP,cTnI and CK-MB combined detection can be used as the sensitivity in the early diagnosis of AMI.
