Non-invasive positive pressure ventilation%Bronchoscopy%Pulmonary heart disease and pulmonary encephalopathy
目的:研究分析无创正压通气联合纤维支气管镜吸痰治疗肺心病并肺性脑病的临床效果。方法选取2012年1月~2014年1月收治的肺心病并肺性脑病患者60例进行回顾性分析,将其分为观察组和对照组,其中对照组30例,采取常规治疗方法,观察组30例,在对照组基础上采取无创正压通气联合纤维支气管镜吸痰治疗,对治疗前、中、后行实验室指标、生命体征、脉搏、血氧饱和度、血气分析等观察监测,并作分析比较。结果观察组患者治疗前、后PaCO2分别为(11.04±2.15)kPa和(7.84±1.69)kPa,PaO2分别为(7.80±1.88)kPa和(11.13±2.08)kPa,通气后患者病情有显著改善(P<0.05);与对照组比较,观察组的痰菌阳性例数、死亡例数、住院天数均有明显的优势,差异均有统计学意义。结论无创正压通气联合纤维支气管镜吸痰治疗肺心病并肺性脑病的临床效果相对比常规治疗的效果较好,值得临床推广使用。

목적:연구분석무창정압통기연합섬유지기관경흡담치료폐심병병폐성뇌병적림상효과。방법선취2012년1월~2014년1월수치적폐심병병폐성뇌병환자60례진행회고성분석,장기분위관찰조화대조조,기중대조조30례,채취상규치료방법,관찰조30례,재대조조기출상채취무창정압통기연합섬유지기관경흡담치료,대치료전、중、후행실험실지표、생명체정、맥박、혈양포화도、혈기분석등관찰감측,병작분석비교。결과관찰조환자치료전、후PaCO2분별위(11.04±2.15)kPa화(7.84±1.69)kPa,PaO2분별위(7.80±1.88)kPa화(11.13±2.08)kPa,통기후환자병정유현저개선(P<0.05);여대조조비교,관찰조적담균양성례수、사망례수、주원천수균유명현적우세,차이균유통계학의의。결론무창정압통기연합섬유지기관경흡담치료폐심병병폐성뇌병적림상효과상대비상규치료적효과교호,치득림상추엄사용。
Objective Research and analysis of the clinical effect of noninvasive positive pressure ventilation combined bronchoscope sputum aspiration in treating pulmonary heart disease complicated with pulmonary encephalopathy. Methods Pulmonary heart disease from 2012 January~2014 year in January were complicated with pulmonary encephalopathy in 60 cases were retrospectively analyzed,which is divided into the observation group and the control group, including 30 cases of the control group,took conventional treatment,the observation group 30 cases,on the basis of the control group taking noninvasive positive pressure ventilation combined with fiber bronchoscope treatment of sputum aspiration,before treatment,after in,laboratory index,vital signs, pulse,oxygen saturation,blood gas analysis and observation of monitoring,analysis and comparison. Results The observation group patients before and after treatment,PaCO2 were(11.04±2.15)kPa and(7.84±1.69)kPa,PaO2 were(7.80±1.88)kPa and(11.13±2.08)kPa patients,after ventilation improved significantly(P < 0.05);compared with the control group,the observation group of sputum positive cases the number of the number of cases,death, hospitalization days have obvious advantages,the difference had statistical significance. Conclusion The clinical efficacy of noninvasive positive pressure ventilation combined bronchoscope sputum aspiration in treating pulmonary heart disease complicated with pulmonary encephalopathy is relatively better than conventional treatment effect,is worthy of clinical use.
