胰高糖素样肽-1(GLP-1)类似物是目前用于治疗2型糖尿病较新的一类药物,动物模型和临床研究发现,GLP-1类似物对糖尿病肾病的肾功能也具有一定的保护作用,其机制主要来源于抑制细胞间黏附分子-1,减少巨噬细胞的滤过,抑制NF-κB活动以及抗炎和抗氧化等作用。此外,GLP-1还可以影响肾脏的水和电解质平衡。在安全性方面,尽管也有GLP-1类似物引起肾损伤的个案报道,但是多项研究结果表明,GLP-1类似物在轻中度的糖尿病肾病肾损伤患者中可以安全使用,但是对于重度肾损伤患者包括终末期肾病( ESRD)则不建议使用。

GLP-1 analogy is a new kind of drug for T2DM,its reno-protective effect is confirmed in vitro and in vivo experiments. The major mechanism stems from:suppressing ICAM-1,reducing macrophage infiltration,decrea-sing nuclear factor-κB and anti-inflammation and anti-oxidative effects. In addition,GLP-1 can modulate water and e-lectrolyte balance in the kidney. As the safety is concerned,although there are reports of renal impairment induced by GLP-1 analogy,several meta-studies indicate that GLP-1 analogy can be safely used in mild degree T2DM renal impair-ment,while it is not recommended in patients with severe renal impairment or ESRD.
