目的:探讨亚洲人骨质疏松自我筛查工具(Osteoporosis Self-assessment Tool for Asians,OSTA)在慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)患者骨质疏松风险评估中的应用前景。方法:根据OSTA风险分层,对33例呼吸系统疾病患者,其中COPD23例,进行相关特征的分析。结果:COPD组骨质疏松中、高风险患者所占比例较对照组高。COPD组中不同风险组间在身高差、体重、BMI,甲状旁腺激素(PTH)、同型半胱氨酸(Hcy)水平,任何重要的骨质疏松性骨折(Major osteoporotic)、髋部骨折(Hip Fracture)概率及深吸气量(IC)的差异,存在统计学意义。OSTA与下列因素的相关性存在统计学差异,其中与身高差、Major osteoporotic概率、Hip Fracture概率、PTH、Hcy呈正相关,与体重、BMI、用力肺活量(FVC)、IC呈负相关。结论:COPD患者骨质疏松的风险较高,对于尚不具备双能X线吸收测量法(DXA)检测条件的基层医院,OSTA不失为一种有效的初步筛选工具。

목적:탐토아주인골질소송자아사사공구(Osteoporosis Self-assessment Tool for Asians,OSTA)재만성조새성폐질병(Chronic obstructive pulmonary disease,COPD)환자골질소송풍험평고중적응용전경。방법:근거OSTA풍험분층,대33례호흡계통질병환자,기중COPD23례,진행상관특정적분석。결과:COPD조골질소송중、고풍험환자소점비례교대조조고。COPD조중불동풍험조간재신고차、체중、BMI,갑상방선격소(PTH)、동형반광안산(Hcy)수평,임하중요적골질소송성골절(Major osteoporotic)、관부골절(Hip Fracture)개솔급심흡기량(IC)적차이,존재통계학의의。OSTA여하렬인소적상관성존재통계학차이,기중여신고차、Major osteoporotic개솔、Hip Fracture개솔、PTH、Hcy정정상관,여체중、BMI、용력폐활량(FVC)、IC정부상관。결론:COPD환자골질소송적풍험교고,대우상불구비쌍능X선흡수측량법(DXA)검측조건적기층의원,OSTA불실위일충유효적초보사선공구。
Objective: To evaluate application prospect for OSTA in the osteoporosis risk assessment of COPD patients. Methods:Based on risk stratification about OSTA, a correlation characteristics analysis of 33 cases of patients with respiratory system diseases, 23 cases of COPD inclusive, was conducted.Results:It was observed in COPD group that the proportion of high-risk for osteoporosis was higher than that in control group.In COPD group,The difference in terms of height loss, weight, BMI ,the level of PTH,HCY, Major osteoporotic probability, Hip Fracture probability, Inspiratory capacity among different risk groups was significant statistic-wise. OSTA is associated with the following factors, There was a positive correlation between height loss, the level of PTH,HCY, Major osteoporotic probability, Hip Fracture probability and OSTA. Weight, BMI, FVC,IC are negative correlation.Conclusion:Patients with COPD are at higher risk of osteoporosis than those of other respiratory system diseases. For basic hospitals without the DXA detection equipment, OSTA is an effective preliminary screening tool.
