目的:使用频域光学相干断层扫描(OCT)检测早期原发性开角型青光眼(POAG)视盘参数与视野平均缺损(MD)的相关性,评估频域OCT检测视盘参数在POAG早期诊断中的价值。方法正常人55例(55只眼)及早期POAG患者55例(55只眼)。采用Cirrus SD-OCT和Synemed EP-930自动视野计分别进行视盘形态和视野检测。 OCT检测采用optic disc cube 200×200模式,评估数据包括视盘面积、盘沿面积、平均杯盘比、垂直杯盘比及视杯容积。视野检测采用Synemed EP-930自动视野计30°-2程序。比较2组间视盘参数的差异,分析早期青光眼组MD数值与Cirrus SD-OCT视盘参数的关系。结果早期POAG组与正常组之间的视盘参数比较,除视盘面积外其余参数均存在统计学意义的差异(P<0.001)。早期POAG组盘沿面积与视野MD相关性最好(r=0.561,P<0.001)。结论频域OCT检测的视盘参数在POAG早期诊断中具有重要价值。

OBJECTIVE To study the correlation between optic disc parameters measured by Cirrus SD-OCT and visual field detect in early primary open-angle glaucoma (POAG). METHODS The tomography of optic nerve head was measured by Cirrus SD-OCT in 55 patients (55 eyes) with early POAG and 55 normal subjects 55 eyes. 3D cube OCT data was obtained using Opitic Disk Cube 200×200 Scan pattern, which performed raster scanning in a 6×6mm square centered on the opitic head composed of 200A-scans and 200B-scans. Disc area, rim area, average cup/disc ratio, vertical cup/disc ratio, and cup volume were evaluated. These ONH parameters were compared be-tween normal subjects and eyes with early POAG. All eyes were detected by Synemed EP-930 perimetry. The corre-lation between mean deviation and optic disc parameters in early POAG eyes was also analyzed. SPSS 17.0 statistical software was used for statistical analyses. RESULTS There were significant differences of all optic disc parameters between normal and early POAG eyes except disc area (P<0.001). Rim area had marked correlation with MD in ear-ly POAG eyes (r=0.562, P<0.001). CONCLUSIONS Measurement of optic disc parameters by Cirrus HD-OCT was useful in detecting early glaucomatous damage and had marked correlation with MD.
