目的:探讨反复多次光凝对角膜内皮细胞(CEC)的影响。方法前瞻性非随机临床研究。选择需行全视网膜激光光凝术的患者55例55眼(均选右眼),非接触角膜内皮镜检测角膜中央、12点位及6点位的内皮不同参数。采用重复测量的方差分析及 LSD 检验比较每个参数的变化。结果完成4次激光术后,六角形内皮细胞比例(HEX)在12点位及6点位均有明显改变[12点术前(65.09±18.60)%,术后(63.39±18.39)%,F =1.679;6点术前(64.86±18.48)%,术后(62.95±17.66)%,F=1.978]差异均有统计学意义(P 均<0.05);变异系数(CV)在角膜中央、12点位及6点位均有明显改变[角膜中央术前(26.24±3.99)%,术后(27.00±4.48)%,F=4.122;12点术前(26.19±4.09)%,术后(27.14±4.47)%,F=4.183;6点术前(26.17±4.34)%,术后(27.38±4.18)%,F =4.079]差异均有统计学意义(P 均<0.05);平均细胞面积(AVE)在角膜中央、12点位及6点位均有明显改变[角膜中央术前(387.81±49.92)μm2,术后(404.48±50.59)μm2,F=5.136;12点术前(385.71±46.91)μm2,术后(408.38±53.70)μm2;F=5.566;6点术前(388.85±46.02)μm2,术后(406.04±50.71)μm2;F=6.894]差异均有统计学意义(P 均<0.05);角膜内皮密度(CD)在 4次激光术后各位点均减少,但无统计学意义(P 均>0.05)。结论:反复多次视网膜激光光凝术后角膜内皮在短期内有改变,尤其平均细胞面积和变异系数较明显,其改变和激光能量密切相关。

Objective To analyze the changes of corneal endothelium after repeatedly Retinal laser photocoagula﹣tion. Methods This was a Prospective nonrandomized clinical study, fifty-five patients 55 eyes(pick right one)who un﹣derwent panrentinal photocoagulation were recruited. The cornea was divided into 3 sections s including central, 12 point, 6 point. The Corneal endothelial meter was used to collect CECsˊ patameter data. The data analysis were done separately by the variance analysis and Least-significant difference. Results Comparing the data before and 28 days af﹣ter in 3 sections, there were significant difference(both P<0. 05)in the Hexagonal cell percentage(HEX) (12 point changed from 65. 09±18. 60% to 63. 39 ±18. 39% , F = 1. 679;6 point changed from 64. 86 ±18. 48% to 62. 95 ± 17. 66% , F =1. 978). And there were significant difference(all P<0. 05)in the Coefficient of Variation (CV)(corne﹣a from 26. 24±3. 99% to 27. 00±4. 48% , F =4. 122;12 point from 26. 19±4. 09% to 27. 14±4. 47% , F = 4. 183;6 point from 26. 17±4. 34% to 27. 38±4. 18% , F =4. 079. Moreover, there were significant difference(all P<0. 05)in the average cell area( AVE) ( cornea from 387. 81 ±49. 92μm2 to 404. 48 ±50. 59μm2 , F = 5. 136; 12 point from 385. 71±46. 91μm2 to 408. 38±53. 70μm2 , F =5. 566; 6 point from 388. 85±46. 02μm2 to 406. 04±50. 71μm2 , F =6. 894). However, the cell density(CD) was not statistically significant among 3 sections(all P>0. 05). Conclusion ECEs were changed in a short time after the repeatedly treatment of Retinal laser photocoagulation, especial the Hexa﹣gonal cell percentage and the average cell area, itˊs highly related to laser energy.
