目的:观察老年慢性充血性心力衰竭(CHF)患者脑钠肽(BNP)和胱抑素 C(Cys C)水平与心功能的变化。方法随机选择63例老年 CHF患者,对不同心功能分级的老年慢性 CHF患者血浆 BNP、Cys C水平与左室舒张末期内径(LVD)、左室收缩末期内径(LVS)、左室射血分数(LVEF)、心输出量(CO)进行检测。结果心功能分级Ⅰ级~Ⅵ级的老年慢性 CHF患者,血浆 BNP和 Cys C水平与 LVD和 LVS值逐渐升高,LVEF 和 CO 值则逐渐下降,NYHA心功能Ⅰ级~Ⅵ级患者血浆 BNP、Cys C 水平、LVD、LVS、LVEF和 CO值比较,差异均有统计学意义(P<0.05)。结论老年慢性 CHF患者血浆 BNP和 Cys C水平变化与心功能密切相关,可作为心功能评价和治疗疗效及预后判断的敏感指标,具有较高的临床价值。

목적:관찰노년만성충혈성심력쇠갈(CHF)환자뇌납태(BNP)화광억소 C(Cys C)수평여심공능적변화。방법수궤선택63례노년 CHF환자,대불동심공능분급적노년만성 CHF환자혈장 BNP、Cys C수평여좌실서장말기내경(LVD)、좌실수축말기내경(LVS)、좌실사혈분수(LVEF)、심수출량(CO)진행검측。결과심공능분급Ⅰ급~Ⅵ급적노년만성 CHF환자,혈장 BNP화 Cys C수평여 LVD화 LVS치축점승고,LVEF 화 CO 치칙축점하강,NYHA심공능Ⅰ급~Ⅵ급환자혈장 BNP、Cys C 수평、LVD、LVS、LVEF화 CO치비교,차이균유통계학의의(P<0.05)。결론노년만성 CHF환자혈장 BNP화 Cys C수평변화여심공능밀절상관,가작위심공능평개화치료료효급예후판단적민감지표,구유교고적림상개치。
Objective To observe the changes of plasma brain natriuretic peptide (BNP),cystatin C (Cys C) levels and cardiac func-tion in elderly patients with chronic congestive heart failure(CHF). Methods Sixty three elderly patients with CHF were choosed randomly,and divided into 4 groups according to the New York Heart Association (NYHA) functional classification:13 in group I,16 in group II,19 in group III and 15 in group IV.The changes of plasma BNP and Cys C,left ventricular end diastolic diameter (LVD),left ventricular end systolic diameter (LVS),ejection fraction (EF) and cardiac output (CO) were observed.Results With the changing of cardiac function,the levels of plasma BNP,Cys C,LVD and LVS were increased obviously while LVEF and CO were decreased.That meant the worse cardiac function,the higher plasma BNP and Cys C level,which was of statistical significance(P<0.05).Conclu-sion Plasma BNP and Cys C levels and cardiac function are closely related to cardiac function in elderly patients with CHF,which can be used as sensitive index of the evaluation of cardiac function,treatment effect and prognosis.
